In this role, you sit at the center of that work. You help patients move from intention to follow-through, and you help the care team translate medical goals into sustainable behavior change.

Because you are embedded with physicians, health coaches, and the broader care team, your work does not live in a silo. Your assessments, interventions, and communication shape how the whole team supports each patient’s goals across diet, exercise, stress management, and overall health.

When you do this well, patients feel understood, supported, and capable, and that belief becomes part of their care. Your presence makes the model real.

What You'll Be Doing

You deliver behavioral health care that is fully integrated into True Health’s patient-centered model.

You assess what is getting in the way and build a plan that fits

· Conduct comprehensive psychological assessments focused on behavioral, emotional, and motivational factors.

· Create individualized behavioral health treatment plans aligned with physician-directed goals and Lifestyle Medicine programming.

· Provide evidence-based interventions including CBT, MI, ACT, and behavioral activation techniques.

· Track progress, adjust treatment plans, and share outcomes with the coordinating physician and care team.


You help patients turn motivation into sustainable behavior change

· Identify and address barriers like stress, emotional eating, avoidance behaviors, and negative self-talk.

· Guide patients through evidence-based behavior change frameworks that support real-world follow-through.

· Build resilience, self-efficacy, and internal accountability with patients over time.

· Partner with health coaches to reinforce behavioral strategies across the patient’s wellness programming.


You strengthen mindset, goal alignment, and follow-through over time

· Help patients clarify intrinsic motivations and connect them to daily choices.

· Address patterns like fixed mindset, fear of failure, and perfectionism that undermine engagement.

· Teach tools for navigating setbacks, plateaus, and the emotional complexity of sustained change.

· Support patients in developing a healthier identity and relationship with their own well-being.


You integrate seamlessly with the care team and uphold clinical standards

· Participate in care team consultations and patient care conferences to align behavioral and medical goals.

· Communicate consistently with physicians, health coaches, and other team members on progress and clinical concerns.

· Contribute to integrated care protocols that connect behavioral health with Lifestyle Medicine, nutrition, and physical therapy.

· Document accurately and on time in the EMR in line with True Health standards and HIPAA requirements.


You deliver a concierge-level patient experience

· Bring warmth, empathy, and professionalism to every patient interaction.

· Create a safe, supportive, non-judgmental environment for honest clinical work.

· Educate patients on how psychological well-being and physical health connect within an integrative care plan.

What You Bring

You're someone who:

· Makes patients feel safe enough to be honest, even when the topic is hard.

· Holds structure and compassion at the same time, keeping sessions grounded and goal-connected.

· Communicates clearly with a multidisciplinary team so care stays coordinated and consistent.

· Stays steady and emotionally present across a full day of patient sessions.

· Follows through on documentation and clinical standards without letting details slip.


Experience that helps:

· Doctoral degree (Ph.D. or Psy.D.) in Clinical Psychology, Health Psychology, or a closely related field from an accredited program, with a minimum of a master’s degree.

· Active and unrestricted Psychologist licensure in the state of Illinois.

· Experience delivering evidence-based behavior change interventions such as CBT, MI, ACT, or equivalent approaches.

· Proficiency with EMR systems and clinical documentation standards.

While this position is based in Deerfield, it serves both Deerfield and Buffalo Grove locations and you must have reliable transportation to travel between the two offices.
