Job Responsibilities – Process home health referrals by contacting clinicians promptly and accurately – Collaborate with pod team members to ensure timely Start of Care (SOC) and Resumption of Care (ROC) – Track all pending start-of-care patients and follow up consistently – Report non-admit cases to the Intake Supervisor/Administrator – Serve as a liaison between the company and patients/caregivers – Communicate with referral sources and sales team regarding new referrals – Explain company programs and services effectively – Learn and retain medical terminology and protocols – Perform electronic filing and general administrative tasks – Complete additional duties assigned by supervisor
Qualifications – Strong written and verbal communication skills – Excellent customer service and professional demeanor – Detail-oriented with strong organizational abilities – Ability to multitask and manage competing priorities – Ability to work independently and in a team environment – Proficient in Microsoft Office Suite – Accurate documentation skills – Spreadsheet preparation and tracking experience – Presentation and administrative writing skills – Flexible and cooperative attitude
Education & Experience – Associate’s Degree preferred; Bachelor’s Degree or equivalent work experience accepted – Minimum 1 year of home health care experience preferred – At least 1 year of administrative support experience required – Familiarity with medical terminology preferred – Scheduling experience a plus
