Life Redesign Coach

CONNECT is hiring Life Redesign Coaches to support individuals in building meaningful, independent, and connected lives. Using a “doing with” approach, you’ll partner with people to help them achieve their goals and actively participate in daily life and community.

This is a hands-on, relationship-focused role where you’ll make a real difference every day.

Coaching & Communication

- Build strong, respectful relationships

- Support individuals in identifying goals and strengths

- Provide encouragement, guidance, and emotional support

- Communicate with families and team members

- Document updates using digital systems

Daily Living Support

- Assist with hygiene, grooming, and dressing

- Support meal preparation and nutrition

- Assist with mobility and physical activities

- Administer medications and attend appointments

- Promote mental wellness and healthy routines

- Maintain a clean and safe environment

Home & Lifestyle Support

- Create a welcoming, home-like environment

- Prepare nutritious meals

- Support household tasks and organization

Community & Social Engagement

- Help individuals build relationships and connect with their community

- Plan and participate in activities

- Support transportation (driving, transit, taxis, HandiDART)

- Strong communication and interpersonal skills

- Compassionate, patient, and adaptable

- Team-oriented with a positive attitude

- Comfortable supporting personal care needs

- Problem-solver with a flexible mindset

- Criminal Record Check (Vulnerable Sector)

- First Aid & CPR

- Food Safe Certificate

- TB test (within 30 days)

- Immunization declaration

- Valid driver’s license + driver’s abstract

- Ongoing training and coaching
