Position Description:
The Population Health Preventive & Transitional Care Supervisor (LPN) provides clinical and operational leadership for the organization's preventive health and transitional care programs. This position oversees Annual Wellness Visits (AWV), Transitional Care Management (TCM), preventive outreach initiatives, and post-discharge workflows that improve preventive care delivery, reduce avoidable readmissions, and identify patients who would benefit from ongoing Population Health services.
The Supervisor leads efforts to optimize preventive care processes by supporting value-based initiatives, strengthen transitions of care, improve patient access, and ensure eligible patients are connected to appropriate longitudinal Population Health programs. This role serves as the operational bridge between episodic patient encounters and long-term care management.
Responsibilities:
- Provide day-to-day leadership and supervision for assigned Population Health staff.
- Provide operational oversight of Annual Wellness Visits (AWV), Transitional Care Management (TCM), and preventive value-based health programs.
- Lead development and optimization of preventive outreach workflows, discharge management processes, scheduling coordination, documentation standards, and operational protocols.
- Monitor preventive care performance, transitional care outcomes, and patient engagement metrics.
- Collaborate with leadership to improve patient identification, preventive service utilization, and care gap closure.
- Develop standardized referral pathways from AWV and TCM into appropriate Population Health longitudinal care management programs.
- Develop collaborative relationships with providers, practice leadership, hospital care coordination, care management staff, and ACO partners for integrated workflows and services.
- Promote effective communication surrounding hospital discharges, preventive care opportunities, and patient follow-up.
- Monitor quality, productivity, documentation, and operational performance for assigned programs.
- Ensure compliance with organizational policies, regulatory requirements, and payer guidelines.
Qualifications:
- Required
- Current New York State Licensed Practical Nurse (LPN) license in good standing.
- Active Basic Life Support certification.
- Minimum of five (5) years of progressive clinical experience.
- One (1) to three (3) years of supervisory or leadership experience in a healthcare setting.
- Experience in preventive health services, ambulatory care, transitional care, care coordination, or Population Health.
- Demonstrated leadership, communication, organizational, and problem-solving skills.
- Ability to effectively collaborate with providers, leadership, clinical staff, and interdisciplinary teams.
- Proficiency with electronic health records and Microsoft Office applications.
- Preferred
- Experience with Medicare preventive services, transitional care programs, and Population Health initiatives.
- Experience within integrated teams and implementing workflow improvements.
- Experience with quality improvement, accreditation activities, and practice transformation initiatives.
Pay Range: $62,354 – $73,000.00
