Intake Nurse - VNA in Watertown, Connecticut at Waterbury Hospital

JOB SUMMARY:

• Receives and processes referral intake information
• Demonstrates the ability to obtain and track Face to Face (F2F) documentation for applicable referrals
• Provides exemplary customer service to all referral sources
• Adheres to HIPAA Policies and Procedures
• Performs all other duties that Management may determine necessary
• Reports to Clinical Manager

QUALIFICATIONS:

Minimum Education and Experience

• LPN license in good standing in the State of Connecticut
• One year experience as an LPN in an acute/sub-acute or home care setting
• Proficiency in the use of computers and software applications

Other Qualifications:

• Physical, mental and sensory abilities sufficient to meet performance standards with or without assisted devices
• Effective oral and written communication skills
• Must have good organizational skills and be detail focused
• Ability to work in a fast paced environment with demonstrated ability to handle multiple competing tasks and demands
