Description
Position Summary
- Serves as FGDM facilitator, including preparation, facilitation and follow up for family meetings in the program.
- Collaborates with staff and administration to create a process that encourages the identification of family strengths to resolve family issues and achieve safety, permanency, and well-being for the involved children.
- Coordinates the receipt of referrals, orientation, and preparation of family members, connecting with community resources and setting up family conferences.
- Collaborates with family and stakeholder participants to determine an appropriate action plan.
- Trains and informs staff and community stakeholders in the FGDM model.
- Works with community and professional groups to coordinate, improve and strengthen knowledge between child welfare and external stakeholders.
- Prepares administrative reports to facilitate collection of data associated with the FGDM model to monitor performance outcomes.
- Performs other duties as assigned and required to maintain unit operations.
- Promotes and demonstrates appropriate respect for cultural diversity among co-workers and all work-related contacts.
- Assists case managers in locating identified family members.
- Assists case mangers in looking through case records to identify family members.
Essential Qualifications
- Credentials/Licensure: Valid driver’s license and auto insurance
- Clearances: State Police, FBI, and Child Abuse Clearances; CPSL Mandated Reporter-Recognizing and Reporting Child Abuse training per state regulation.
Working Conditions
- Physical Demands: Requires vision, speech, and hearing and a moderate to high level of physical activity including; sitting, standing, walking lifting and carrying as required. Must be able to meet the demands and complete Crisis Intervention & Prevention (CIP).
- Environmental Factors: Indoors/Outdoors: school, home, community
- Working Hours: Flexible non-traditional office hours; variable schedule based on program need.
