Job Summary

The Licensed Practical Nurse (LPN) is responsible for assessment and direct care of residents including, but not limited to, resident assessments and monitoring, coordination of services, infection control, medication management, treatments, and wound dressing. The LPN will practice within the legal boundaries for practical nursing authorized by the state for the delivery of patient care and related services

Essential Duties and Responsibilities

Resident Assessment

• Adhere to state rules and regulations concerning delivery of care and assures that effective quality nursing care is delivered which is outcome focused through the utilization of the nursing process.
• Demonstrate the ability to perform physical assessment through observation, inspection, palpation, percussion, and auscultation. If applicable, participates in the accurate completion of the RAI co

Develops Plan of Care

• Functions as a member of the health care team, assisting with the development and modification of individualized plans of care in collaboration with the resident/responsible party and other members of the interdisciplinary care team. Conducts appraisals of the resident’s status and situation that contributes to ongoing.
• Implements appropriate nursing interventions consistent with the resident plan of care, plans for episodic nursing care, and documents appropriately in the medical record.
• Implements teaching with resident/responsible party consistent with the interdisciplinary plan of care.

Implements Plans of Care

• Assume responsibility for care interventions consistent with skill level and professional standards of care.
• Keep physician and/or other health care professionals informed of resident’s condition, and notify physician and/or other health care professionals immediately of significant changes of condition.
• Work in collaboration with physicians and/or other health care professionals by sharing information relevant to changing plans of care.
• Administer medication and perform treatment accurately.
• Recognize normal and abnormal lab values and communicate levels to the physician and other interdisciplinary team members as appropriate

Other responsibilities

• Coach, counsel, and assign Certified Nursing Assistants to provide quality resident care. Evaluate the job skills and work performance of the Certified Nursing Assistant.
• Assign Certified Nursing Assistants specific duties for resident care and direct their work.
• Ensure proper staffing by contacting replacement employees; transfer or reassign Certified Nursing Assistants to ensure adequate staffing levels.
• Establish and maintain positive interactions with internal and external customers, including co-workers and direct supports.
• Support resident autonomy and choice. Demonstrate both sensitivity and appropriate responsiveness to each resident’s end of life issues.
• Demonstrate safe practices regarding the resident’s comfort and safety by applying knowledge of proper body alignment for self and resident. Promote a restraint-free environment.
• Demonstrate working knowledge of infection control practices.

Working Conditions

• May be exposed to body fluids or waste, as well as diseases or other conditions.
• Sitting, standing, bending, lifting, and moving intermittently during work hours.
• Must be able to lift a minimum of 35 pounds.
• Subject to frequent interruptions and sometimes hostile and emotionally upset residents
• Must be able to assist in the evacuation of residents.

• Basic computer knowledge and ability with an aptitude to learn company software.
• Must be capable of maintaining regular attendance

Education and Experience

• Must hold and maintain a current license to practice as a Licensed Practical Nurse (LPN) in the state of Washington.
• Recent clinical experience, education, or specialty skills specific to skilled nursing.
