ORTHOPEDIC NURSE NAVIGATOR in Salina, Kansas at Salina Regional Health Center

Under the supervision of the Director or clinic manager in collaboration with clinic physicians, mid-level providers, and surgery schedulers, the Orthopedic Nurse Navigator is an advocate, educator, and experienced guide for the surgical patient, as well as family members, through the total joint replacement continuum. The Orthopedic Nurse Navigator (ONN) will coordinate and oversee all aspects of patient care by working closely with providers, patients, and families pre-operatively, post-operatively, and post-discharge to ensure that patients receive timely high-quality care. The ONN assumes responsibility and accountability for optimal clinical outcomes and coordination of care activities for patients receiving orthopedic services, through joint replacement, general orthopedic surgery, and fracture care programs. The ONN serves as a consistent care coordinator ensuring timely access and seamless coordination of services throughout the continuum of total joint care. The ONN will provide medical information, education, and share knowledge regarding internal and external support services and resources. Will be responsible for facilitating: scheduling, coordinating follow-up care, and tracking patients to ensure completion of the recommended treatment plan. Will identify and assist patients and families in eliminating potential barriers to care by identifying and referring to appropriate resources. Performs as a positive member of the healthcare team.

Graduation from an accredited school of nursing/licensed practical nursing program or completion of a medical assisting training course

6 months clinical experience

Orthopedic experience preferred

Medical Assisting Certification (CMA, CCMA, NCMA, or RMA) or Licensed Practical Nurse (LPN)

Basic Life Support certification successfully completed within the first 90 days of employment
