Preferred Care Home Health is seeking a Full-Time Registered Nurse (RN) to provide skilled home health care to patients in assisted living, independent living, and other residential care communities in the Fort Myers, FL Area

• Perform comprehensive nursing assessments and admissions for home health patients
• Develop and manage individualized plans of care alongside physicians and the care team
• Deliver skilled nursing visits per physician orders and agency policy
• Monitor patients closely and report changes in condition promptly
• Educate patients, families, and caregivers on disease management, medications, and safety
• Complete OASIS assessments and documentation accurately and on time
• Coordinate with therapists, social workers, physicians, and facility staff
• Maintain compliance with Medicare, CMS, and ACHC standards
• Participate in case conferences and interdisciplinary team meetings

• Graduate of an accredited nursing program
• Active, current Florida RN license
• Current CPR/BLS certification
• Minimum 5 years of RN experience
• Home health and OASIS documentation experience preferred
• Strong assessment, communication, and organizational skills
• Comfortable working independently while collaborating with a team
