PURPOSE:

The Switchpoint Case Manager Supervisor provides direct supervision to the Switchpoint Case Management Team in Harris Apartment, and is primarily responsible for program development, implementation, and fidelity. Data collection and validation, licensing and compliance corresponding to the case management program, especially where housing prioritization is concerned, are also responsibilities of the Case Manager Supervisor.

The Case Manager Supervisor has primary responsibility for coordinating all case management and housing services under the general direction of the Case Management Compliance Manager

JOB RESPONSIBILITIES:

· Develop, implement, and maintain fidelity of all Switchpoint case management programs in your Switchpoint locations.

· Review all data, files, case notes, etc. pertaining to case management at each Switchpoint location – provide critical feedback to corresponding case management teams.

· Work with Case Management Compliance Manager to review all data for state and grant reporting.

· Work with Case Management Compliance Manager and the Utah State Office of Licensing to ensure all Switchpoint locations’ licenses are renewed annually and are in compliance with state regulations.

· Serve as a point of contact for community resource organizations.

· Maintain professional knowledge in applicable areas, including Fair Housing Laws.

· Participate in regular staff meetings, staff training, supervisor sessions and accept responsibility for aiding the development of positive team relationships as requested.

· Adhere to agency policy, procedures, safety standards and professional code of ethics.

· Assist in the implementation of community and social programs in accordance with grants/contracts with federal, state and local agencies and mandates of service requirements with those grants/contracts.

· Coordinate with clinical, health education, community health and other staff members in the provision of services to clients. Provide referrals for services as needed.

· Coordinate with other organizational entities involved in human services and housing services.

· Provide in-service presentations/trainings to staff as needed.

· Provide community resource service information as an outreach worker as required.

· Be a team player and assume other responsibilities as directed.

· Other duties as assigned.

WORK IN CONJUNCTION WITH RESPECTIVE CASE MANAGER SUPERVISORS TO ENSURE THE FOLLOWING CASE MANAGEMENT RESPONSIBILITIES ARE BEING ACHIEVED:

Housing:

· Works in conjunction with other Case Management Compliance Manager to coordinate and implement Life Skills, Tenancy 101, On-The-Job Training and other training programs aimed at learning and practicing life skills and decision-making

· Works in conjunction with Case Management Compliance Manager for on-going program evaluation of effectiveness and producing changes in client behaviors

· Interviews and counsels' prospective clients for the Supportive Housing Programs Switchpoint Offers

· Maintain case load as directed, helping clients accomplish their goals and work toward self-sufficiency; offer counsel and guidance; maintain regular face-to-face contact with clients.

· Develops a comprehensive client-driven case plan with both short-term and long-term goals identified; Completes a timeline and measures for each goal

· Evaluates and adjusts case plans as needed and provides written warnings with consequences if appropriate progress is not being met.

· Empowers clients to appropriate resources to assist with meeting goals

· Assures that specific HUD program goals are consistently being met and provides extensive case work to foster transition from homelessness to permanent housing

· Maintains client files to include important conversations, warnings, progress towards goals and documentation of any incidents

· Monitor family progress through phone calls, site visits and case management appointments. Advocate for needed treatment and services for clients. Act as a liaison between landlords, court/criminal justice system and treatment services; maintain ongoing contact with service providers. Provide crisis intervention as needed

· Collects data necessary to meet funding requirements and statistical reports

· Participates in trainings deemed necessary for the position

· Reports critical incidents immediately to the Case Manager Program Director, Regional Directors, and Executive Director

CASE MANAGER SUPERVISOR RESPONSIBILITIES ALSO INCLUDE:

Supervisory:

· Monitors the performance of all case management staff and provides coaching to improve their ability to manage situations.

· Assures that case managers receive all the training necessary to meet expectations

Teamwork and Collaboration:

· Works in collaboration with program and other agency staff to facilitate a team environment

· Serves on committees as requested

· Participates in LHC team meetings as requested

· Role models effective team behaviors

· Demonstrates effective communication skills in building relationships with all employees and clients

· Creates good working relationships with local welfare administrators and other area service providers, support groups, non-profits to facilitate access to area resources for clients

· Substitutes for other staff if/when need arises

JOB REQUIREMENTS:

· Computer skills including Microsoft Office, UHMIS, and ability to learn new programs.

· Experience in Human Services and with a variety of populations (mentally ill, disabled, substance abusers, etc.)
