Affinity is urgently seeking reliable RNs and LPNs to provide shift care for 2 hours per day in the 19132 zip code of Philadelphia. We are looking to staff this case as soon as possible.
Responsibilities:
1. Provide skilled nursing care for patients in the home
2. Record patient medical history accurately
3. Take and record vital signs (blood pressure, temperature, heart rate, etc.)
4. Observe patients for progress, medication side effects, and changes in condition
5. Monitor fluid intake/output and maintain accurate patient charts
6. Administer medications, injections, wound care, and other basic medical tasks
7. Collect samples for lab testing as assigned
8. Communicate with PCP and other care team members as needed
9. Provide emotional and psychological support to patients and families
10. Offer guidance and instruction to family members when appropriate
Requirements:
1. Active LPN or RN license in Pennsylvania
2. Experience with trach/vent care preferred (if applicable to the case)
3. Ability to work independently and as part of a multidisciplinary team
4. Strong communication and interpersonal skills
5. Physically able to perform all nursing duties
6. Compassionate, reliable, and able to manage stress in a home care environment
7. Familiarity with Axxess or HHAeXchange is a plus
