Position Summary
The Certified Nursing Assistant (CNA) provides personal care and supportive services to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with the patient’s plan of care, physician orders, agency policies, and applicable state regulations.
This role focuses on assisting patients with activities of daily living, promoting comfort and safety, supporting independence, and observing/reporting patient condition changes to the supervising nurse.
Reporting Structure
This position reports to the Registered Nurse (RN) or designated clinical supervisor. The CNA functions as part of the interdisciplinary care team and provides supportive care within the established plan of care under RN supervision.
Scope of Practice
The CNA performs duties within the scope of Florida CNA certification, agency policies, and applicable state and federal regulations. This role does not provide skilled nursing services, perform clinical assessments, administer medications, or make independent clinical decisions.
Essential Job Functions
1. Personal Care & Daily Living Support
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility
- Provide safe transfers, positioning, and ambulation assistance
- Assist with feeding and hydration as needed
- Help maintain a clean, safe, and comfortable home environment
2. Patient Comfort & Support
- Provide companionship and emotional support to patients and families
- Reinforce self-care, safety, and comfort measures as directed by the supervising nurse
- Support patient dignity, privacy, and independence during care delivery
3. Observation & Communication
- Observe and report changes in patient condition, behavior, safety concerns, or home environment to the supervising RN
- Take and record vital signs, weight, and other basic observations as assigned and permitted by agency policy
- Communicate patient concerns promptly and accurately
4. Documentation & Compliance
- Document care provided accurately and timely according to agency standards
- Follow infection control, safety, HIPAA, and compliance standards at all times
- Maintain confidentiality of patient information and agency records
5. Team Collaboration
- Work closely with nurses, therapists, and other care team members to support continuity of care
- Participate in care coordination and required staff meetings
- Attend required in-services, training sessions, and competency reviews
Qualifications
- High school diploma or equivalent required. Completion of a state-approved Certified Nursing Assistant (CNA) training program required. Current CNA certification in the state of Florida required.
- Minimum of 1 year of patient care experience required
- Home health, hospice, long-term care, or hospital experience preferred
- Current CPR certification required
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to pass background screening and all compliance requirements

Physical Requirements
- Frequent travel to patient homes within assigned service area
- Ability to stand, walk, bend, stoop, kneel, and lift as needed
- Ability to assist with patient transfers and repositioning
- Physical stamina to provide direct patient care throughout the workday
- Ability to work safely in homes with varying accessibility conditions
Work is performed in patient homes and community settings with exposure to variable environments, including pets, smoke, infectious diseases, and weather-related travel conditions.
