Full-Time | Non-Exempt | Helena
Position Overview: Provide mental health services for individuals with severe and chronic mental illness.
Job Duties:
- Works as part of a multi-disciplinary team to help clients develop a personal recovery plan.
- Identify opportunities to provide support as clients work to achieve their recovery goals.
- Serve clients on the PACT team who have a diagnosed SDMI.
- Assist clients with daily living functions in their homes and in the community.
- Meet with treatment team to develop effective support system, outreach, and follow-up.
- Assists clients in menu planning, grocery shopping, socialization skills, personal hygiene skills, housekeeping skills and doing laundry (client’s responsibility with verbal assistance).
- Must pass medication certification test. Performs medication delivery/observation.
- Works rotating on-call schedule for one week at a time.
- On-call duties include providing some clients medications on weekends and carrying on-call phone in case of client crises, providing guidance/direction/assistance to clients should needs arise during non-business hours that week.
- Maintains assigned caseload, based on client and community needs for the PACT Team concept; caseloads will be assigned by the Team Lead.
- Maintains required paperwork for assigned clients in accordance with MRWH policy and WICHE fidelity-based model.
- Helps clients set and achieve meaningful goals in the areas of mental health recovery, education, daily living skills, substance use, relationships, productivity, and leisure.
- Assumes responsibility for the workload assigned, working independently and cooperatively with the team.
- Acts as an advocate for mental health issues in the MRWH region.
- Maintains records, reports, and statistics necessary for program functioning and evaluation.
- Participates in program meetings, employee education and orientation, which will include a close-working relationship with multi-disciplinary team.
- Comply with relevant legislation, regulations, and mandatory guidelines/fidelity of this evidence-based model.
- Responds to emergency health hazards as defined by the Chief Medical Director or Supervisor.
Minimum Qualifications:
- Required graduation from High School or GED and 2 years of experience in the mental health field. A desire to work with people with behavioral health challenges is essential.
- Excellent written skills, computer skills and customer service skills.
- MT Driver’s license and vehicle required. Travel required; will transport clients.
- Must have a personal vehicle, valid insurance, and valid driver’s license.
Wage: $17.94 (includes $1.44 PACT Differential) + DOE
