Job Summary:
We are currently seeking a full-time Psychiatric Nurse Care Coordinator to work Monday-Friday for the Suffolk County House of Corrections. The Coordinator is a registered nurse with specialized knowledge and experience in psychiatric care who supports the coordination, continuity, and safe delivery of psychiatric services within the correctional facility.

Responsibilities:

  • Maintain working familiarity with the active psychiatric caseload and relevant nursing, medication, and continuity-of-care needs.
  • Work closely with the Mental Health Director and psychiatric provider to support the development, organization, and ongoing management of the psychiatry clinical workflow.
  • Contribute nursing, medication, medical, and continuity-of-care information that may affect psychiatric scheduling, clinical priority, or the need for expedited provider review.
  • Coordinate timely verification and continuity of psychiatric medications following admission, transfer, hospitalization, or other transitions of care.
  • Monitor psychiatric medication-related needs, including clinically significant treatment interruptions or adherence concerns, required laboratory monitoring, medication availability, and other issues requiring follow-up.
  • Coordinate the collection and availability of relevant clinical information, including outside psychiatric and medical records, medication histories, laboratory results, and other documentation necessary to support psychiatric evaluation and treatment.
  • Support readiness for psychiatric evaluation by identifying outstanding nursing, medical, mental health, laboratory, records, or other clinical needs and coordinating with the appropriate discipline to address them.
  • Serve as the primary nursing point of contact for routine psychiatric medication concerns and other psychiatric nursing issues identified by facility nursing staff.
  • Facilitate appropriate communication of psychiatric concerns through established clinical pathways and reduce reliance on informal or ad hoc requests to psychiatric providers.
  • Coordinate follow-through on psychiatric treatment plans, orders, monitoring requirements, and other identified needs in collaboration with nursing, mental health, medical providers, pharmacy, and the psychiatric provider.
  • Serve as a liaison among the psychiatric provider, Mental Health Director, mental health clinicians, facility nursing staff, medical providers, pharmacy, and outside healthcare organizations.
  • Promote closed-loop communication and follow-up to ensure that identified psychiatric care needs are addressed and unresolved issues are appropriately escalated.
  • Identify recurring barriers, gaps, or systems issues affecting the delivery of psychiatric care and communicate these concerns to appropriate clinical leadership.

Role Relationships and Boundaries:

The Psychiatric Nurse Care Coordinator works in close partnership with the Mental Health Director and psychiatric provider. The Mental Health Director maintains responsibility for the behavioral health referral process and identification and prioritization of behavioral health clinical needs. The Psychiatric Nurse Care Coordinator contributes psychiatric nursing, medication, medical, and continuity-of-care information to this process and assists in developing and maintaining an organized psychiatric care workflow.

The Psychiatric Nurse Care Coordinator does not replace the responsibilities of facility nursing staff. Routine nursing care, medication administration, and other general nursing functions remain the responsibility of assigned facility nurses.

The Psychiatric Nurse Care Coordinator does not independently diagnose psychiatric conditions, prescribe or modify psychiatric medications, or establish psychiatric treatment plans. Treatment decisions remain the responsibility of the psychiatric provider.

Schedule:

  • Mon-Fri 40 Hours per Week

Salary:

  • $55.00 - $63.00 per hour depending on years of license and shift
Requirements:

Qualifications and Education:

  • Current, unrestricted license as a Registered Nurse.
  • Clinical experience caring for individuals with psychiatric and behavioral health conditions.
  • Knowledge of psychiatric medications, medication monitoring, treatment adherence, and continuity-of-care considerations.
  • Demonstrated ability to coordinate care across multiple disciplines and healthcare settings.
  • Strong organizational skills and ability to manage multiple concurrent and time-sensitive clinical needs.
  • Effective written and verbal communication skills.
  • Ability to collaborate effectively with nursing, mental health, medical, pharmacy, and psychiatric providers.

Preferred Qualifications

  • Psychiatric-mental health nursing experience.
  • Correctional healthcare experience.
  • Experience working with individuals with serious mental illness.
  • Experience in psychiatric care coordination, case management, inpatient psychiatry, forensic mental health, or another setting involving complex psychiatric care.
  • Psychiatric-Mental Health Nursing Certification or other relevant specialty certification.

Physical Requirements:

  • Regularly required to stand; sit; use hands to handle or feel; reach with hands and arms; climb or balance; stoop; kneel, crouch, or crawl; talk and hear.
  • Regularly lift and/or move up to 25 pounds; occasionally lift and/or move over 35 pounds.
  • Regularly able to adequately perform CPR.
