Job title: Licensed Practical Nurse (LPN)
Part-time & Full-Time opportunities on the Evening shift (3:00 - 11:30 pm)
Pay: LPN - $31.00 - $35.00 per hour; Pay is commensurate with licensure. Generous extra pay for evening and weekend shifts.

An LPN protects, nurtures, and sustains life for elders living in their households by providing exceptional personal care, supporting leisure, maintaining a safe, clean house, supporting meals and personal laundry needs, and providing nursing care to meet the needs of the elders according to their care plan for those who live in the home.
The LPN is a member of a team in a small home that work together to create real home reflective the elders who live there.
Qualifications & Education Required: What is required to be successful on the job?
- Graduate of an approved school of practical nursing with current New York State Practical Nursing License required
- CPR certification required
- Valid NYS Driver’s License preferred (required for Green Houses)
- Long-term care experience preferred
- Good organizational, leadership and communication skills
- Desire and ability to interact with elders and care for their needs

Position Responsibilities: What will I be doing to make an impact?
- Responsive to elder needs and preference. Consider elder contentment a priority
- Promotes and supports autonomy of elders in decision making
- Contributes to the nursing process and implements elder’s plan of care
- Safely performs direct nursing services such as administering medications and treatments, providing personal care and activities of daily living
- Actively participates as member of the care team, communicating with needed team members and documenting as needed
- Meets personal care needs of elders
- Consults and validates data with the RN, records and reports pertinent information, provides explanations to families and other staff
- Cares for personal belongings such as clothing, dentures, eyeglasses, hearing aids, prostheses
- Recognizes and reports safety hazards and elders’ health problems and responses to treatment
- Assists with therapeutic and diagnostic procedures
- Plans and supports activities for people living in the home
- Maintains privacy and confidentiality of elder care
- Prepares and serves meals and snacks based on menu and preferences of those living in the home
- Innovates and works autonomously to improve elders’ life
- Actively promotes the Small Home philosophy through behaviors and communications
- Helps with other duties as needed that support St John’s mission vision and values
Physical Requirements:
- Stands, sits, and walks most of the day. Frequent moving and ability to lift up to 50 lbs. Ability to perform activities as defined in skills check list.

- May be exposed to infectious diseases, odors and psycho-behavioral situations
