Job Description
The Social Worker, PATH (Patient Access to Health) serves as the primary psychosocial resource and subject matter expert supporting patients, caregivers, PATH Care Coordinators, providers, and interdisciplinary care teams. This position is responsible for assessing, addressing, and mitigating psychosocial, behavioral health, caregiver, financial, environmental, and social determinants of health (SDOH) barriers that may negatively impact patient outcomes, healthcare utilization, and overall well-being.
Conducts comprehensive psychosocial and biopsychosocial assessments, develops individualized intervention plans, coordinates community resources, facilitates behavioral health and social service referrals, advocacy, and resource navigation.
Functions as the primary escalation point for PATH Care Coordinators and serves as a consultant regarding complex psychosocial situations, behavioral health concerns, SDOH needs, community resource availability, and social service program navigation. Collaborates closely with interdisciplinary team members, providers, community organizations, and caregivers to promote continuity of care, improve access to services, reduce barriers to care, and support health equity initiatives.
SPECIFIC SKILLS NEEDED
• High level of communication skills demonstrating courtesy and professionalism.
• Excellent organizational skills, problem solving, and decision-making abilities.
• Knowledge of healthcare terminology, insurance benefits, referrals, and authorization processes.
• Strong attention to detail and ability to manage multiple tasks in a fast-paced environment.
• Proficient in Windows, Microsoft Word, and Excel.
• Minimum typing speed of 40 wpm.
• Knowledge of medical terminology.
EDUCATION/EXPERIENCE/TRAINING
Required:
• Master's Degree in Social Work
• Minimum of two years of experience in health care field/social services experience
Preferred:
• Licensed Clinical Social Worker
• Experience in customer service, case management, managed care, or healthcare coordination
• Spanish Speaking
• Knowledge of utilization/case management processes and/or health insurance related information
