Job Description
Community Health Worker

Work location: Primarily in the Texas Health Dallas area
Work hours: Monday-Friday from 8:00am-4:30pm
This is a grant funded position and subject to the award and continuation of the contract

Here's What You Need
- H.S. Diploma or equivalent is required. Successful completion of a Community Health Worker formal training program such as from a community college or other education institution is preferred.
- Six (6) months customer service experience either face-to-face or by phone is required, bi-lingual a plus.
- BCLS - Basic Cardiac Life Support is required within 6 months of hire.
- Six (6) months of public health experience preferred
- Community health worker certification with the state of Texas is preferred.

What You Will Do
Coordinate Community Health Improvement Activities
- Participate in program development, planning and evaluation.
- Develop and effectively manage program timelines to ensure timely completion of program deliverables.
- Monitor program activities and reporting to ensure accuracy in outcome measures.
- Facilitate small- and large-group education.
- Establish and manage relationships with community members, organizations and coalitions.
- Identify strategic partnership opportunities and promote relationship-building.
- Disseminate programmatic and marketing materials.
- Develop presentations and other materials for use during program-related events.
- Serve as programmatic liaison to internal and external stakeholders.
- Build and maintain collaborative relationships with FQHC and other low-cost clinics to support care coordination 50%
- Patient Support
- Responsible for establishing trusting relationships with patients and families.
- Navigates patients and families through medical and non-medical support services.
- Conducts eligibility, enrollment and follow up for referred medical or non-medical support.
- Helps patients set personal care plans and attend appointments.
- Works closely with medical care team to ensure coordinated care.
- Service as liaison to internal and external resources through TH network.
- Provides cross coverage support across entities and assisting fellow CHWs to ensure continuity of care.
