Job description:
This position is responsible for fielding telephone calls from physicians, patients and clinical staff ensuring high-quality patient care in compliance with Title 22, Medicare, State licensure regulations, accreditation guidelines, and the organization's policies and procedures.
What You'll Do:
- Responsible for providing skilled nursing intervention and support care via the telephone in accordance with the hospice plan of care for each patient. Assesses the needs of patient and family, determine if a home visit is needed. If a visit is needed, obtains orders and notifies the appropriate Skilled Nurse.
- Interacts with Director of Patient Care Services-Hospice, Patient Care Coordinators, Volunteer and Bereavement Coordinator and Chaplains to support delivery of patient services and compliance with agency policy and all applicable regulations.
- Acts as a contact person for physician orders in the absence of the assigned Case Manager/Clinician. The Case Manager/Clinician is required to call the RN Triage Nurse to be advised of the expected call from the physician.
- Coordinates supplemental orders to promote clinical practices that meet established Livingston nursing care standards. Communicates supplemental orders and patient concerns to appropriate departments so that orders can be carried out and cases assigned and scheduled.
- Interacts and assists with the development and implementation of standards and quality improvement mechanisms that assure safe and quality service to patients and their families.
- Ensures compliance with state, federal, and accreditation regulations regarding hospice care.
- Assists in orientation and ongoing education to ensure adequate qualified staff to meet patient care needs and cooperates and collaborates with DOPCS-Hospice in planning, recruiting and hiring efforts.
- Ensures that customer needs are identified and met productively and positively.
- Initiates the Unusual Occurrence Report for avoidable or potentially avoidable events.
- Keeps the Director of Patient Care Services informed of significant and unresolved patient and family issues.
- Will work as an RN Case Manager providing and documenting intermittent therapeutic, palliative, end-of-life skilled services in the patient's place of residence as needed.
- Performs other duties as assigned.
What We're Looking For:
- Valid California Registered Nurse License
- One year RN Hospice Case Management experience
- Ability to demonstrate skill in nursing practice, ability to cope with family's emotional stress and tolerance of individual lifestyles.
- Knowledge of computer, proficient with Microsoft Word and Excel.
- Good verbal and written communication skills.
- High level of attention to details.
- Able to work independently, take ownership and responsibility of tasks.
- Strong professional ethics, discretion and judgment.
- Good organizational skills.
