SUMMARY:

Provides direct care to residents, treating each with dignity and respect while promoting independence, yet meeting their individual needs in a caring atmosphere.

ESSENTIAL DUTIES/RESPONSIBILITIES:

  1. Represent Independence Care Center in a positive and professional manner.
  2. Know and practice Resident’s rights and exhibit attitudes and behaviors that indicate respect for them.
  3. Demonstrate a caring attitude in all interactions with residents.
  4. Abide by Independence Care Center Personnel Policies and Procedures as outlined in the Employee Handbook.
  5. Know and practice emergency policies and procedures.
  6. Administer nursing care to residents according to the policies and procedures of Independence Care Center.
  7. Utilize all safety equipment and precautions as per policies and procedures when transferring or ambulating residents.
  8. Must use gait belt when transferring or ambulating residents. Must use assist x2 with hoyer lift.
  9. Perform all care using the principles of standard precautions and good hand washing.
  10. Interact with all residents based upon their individual needs.
  11. Maintain a therapeutic environment for all residents; promote and encourage independence.
  12. Report to charge nurse any changes in resident’s condition.
  13. 12 hours of education completed per year.
  14. All other duties as assigned.

CERTIFIED NURSING ASSISTANT- CNA- SHIFT DUTIES

DUTIES FOR 7:00 A.M. to 7:00 P.M. SHIFT:

  1. Receive report from off-going staff and charge nurse.
  2. Review resident’s care plan and administer care, accordingly, based on individual needs.
  3. Be aware of where assigned residents are always and their condition.
  4. Answer all call lights promptly.
  5. Assist with A.M. and P.M. care- partial baths, oral care, incontinent care or toileting, dressing.
  6. Assist all residents with breakfast, lunch, and supper. Feed residents that need assistance as assigned and document intake.
  7. Offer HS snack as indicated and assist as needed.
  8. Pass ice and water as assigned.
  9. Return those residents wishing to take an afternoon nap to bed.
  10. Assist with showers as assigned and sign shower sheet.
  11. Make rounds every two hours- perform toileting, brief changes, incontinent/pericare, ROM exercises, and repositioning of residents.
  12. Perform vital statistics/weights monthly, or as ordered, and document.
  13. Assist with transporting residents to and from scheduled activities.
  14. Perform oral care, nail care, and shaving as needed.
  15. Offer fluids throughout the shift and with rounds.
  16. Straighten resident’s rooms.
  17. Restock linen closets, shower room closets, carts, and pass fresh linens.
  18. Change linens on assigned beds as needed.
  19. Document bowel movements.
  20. Matrix Point of Care charting
  21. Perform duties per assignment sheet and sign off as completed.
  22. Give report to on-coming staff.

DUTIES FOR 7:00 P.M. to 7:00 A.M. SHIFT:

  1. Receive report from off-going staff and charge nurse.
  2. Review resident’s care plan and administer care accordingly, based on individual needs.
  3. Be aware of where assigned residents are always and their condition.
  4. Answer all call lights promptly.
  5. Assist residents in preparing for bed- toileting, partial bath, incontinent care, brief change, nightclothes, and oral care.
  6. Offer HS snack as indicated and assist as needed.
  7. Perform showers, PRN.
  8. Make rounds every two hours- perform toileting, brief changes, incontinent/pericare, ROM exercises, and repositioning of residents.
  9. Beginning at 5:00 a.m. - perform ADL’s, get-ups, assist residents with partial baths, incontinent care, toileting, and dressing.
  10. Perform vital statistics/weights monthly, or as ordered, and document.
  11. Assist with transporting residents to and from scheduled activities.
  12. Perform oral care, nail care, and shaving as needed.
  13. Offer fluids throughout the shift and with rounds.
  14. Straighten resident’s rooms.
  15. Restock linen closets, shower room closets, carts, and pass fresh linens.
  16. Change linens as needed.
  17. Document bowel movements.
  18. Matrix Point of Care Charting.
  19. Perform duties per assignment sheet and sign off as completed.

CREATIVE CARE AIDE- CNA- SHIFT DUTIES

DUTIES FOR 7:00 A.M. to 7:00 P.M. SHIFT:

  1. Finish getting up residents, toilet them, help them get dressed including putting on their TED hose. Make sure teeth are clean and dentures are in mouth. Wash face and glasses, brush hair and put on deodorant.
  2. Clean up tables; wipe off counters, microwave, etc.
  3. Change ice water.
  4. Make beds.
  5. Give shower on shower days, check toenails and ears, shave. Sign shower sheet.
  6. Clean/pick up rooms.
  7. Take out dentures and clean them.
  8. Brush all other teeth.
  9. Make sure TED hose are on.
  10. Feed breakfast, lunch, and supper.
  11. Wash faces, hands, and tables after each meal.
  12. Pass snacks and chart percentages at 1000 and 1400.
  13. Check and toilet ALL residents upon awakening, before and after meals, and PRN; change brief/pull up every shift regardless if wet.
  14. Shave men daily.
  15. Complete Matrix Point of Care Charting.

Any extra time that you have on your shift should be to pamper residents; play games, listen to music, read with them, fix their hair, or just go above and beyond to make them happy and to make them feel loved and cared for.

DUTIES FOR 7:00 P.M. to 7:00 A.M. SHIFT:

  1. You must bring a trash can with you to unit and take it out at the end of your shift.
  2. Do room checks every hour to assure rooms are tidy, clean, and residents are okay.
  3. Check and toilet ALL residents upon awakening, and PRN; change brief/pull up every shift regardless if wet
  4. Change ice water.
  5. Turn off TV to try to get them to go to bed. If you put to bed and they get back up- put in recliner, cover up, and put feet up. Leave TV and lights off. You can sit in dining area if you need light. Put all residents in clean night clothes; remove TED hose and wash them, ring out and hang to dry.
  6. Have them participate in brushing teeth; take out dentures, clean and soak.
  7. Anyone left up at 2300; try to put to bed on rounds. If they won’t go to bed, leave in recliner with feet up and a cover on them. Try again with every round. Leave lights and TV off if in recliners unless they request TV. If you need light, use the dining area.
  8. Clean up rooms; get rid of trash, food, etc.
  9. Take out old wash clothes and hand towels at bedtime.
  10. Wipe off counters and clean up kitchen area.
  11. Change ice water.
  12. Stock cabinets in room.
  13. Complete Matrix Point of Care Charting.
  14. Set up tables for breakfast.
  15. Wake up residents starting at 0530, toilet them, help them get dressed including putting on their TED hose. Make sure teeth are clean and dentures are in mouth. Wash face and glasses, brush hair and put on deodorant.
  16. Clean/tidy up rooms.
  17. Try to have resident to the dining tables by 0700
Requirements:

QUALIFICATIONS:

  1. High school education or G.E.D. preferred.
  2. Must be CNA certified when hired or take CNA course by the time employed 4 months.
  3. Must be at least 18 years of age.
  4. Must be able to read, write, and speak fluently in English.
  5. Must be able to deal with residents and families in a pleasant and supportive manner and must be able to work effectively with fellow employees.

PHYSICAL AND SENSORY REQUIREMENTS:

  1. Must be able to stand, bend, reach, kneel, carry, squat, crawl, lift 75-100 lbs., and move intermittently during working hours using good body mechanics.
  2. To assist residents in a time of emergency must be able to move quickly from one part of the building to another.
  3. Must have good physical health with no communicable diseases present.
  4. Must be able to demonstrate emotional stability in order to cope with the mental and emotional stress of the position.
  5. Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  6. In the event of a resident fall or highly agitated resident, call STAT for help or call the 200 unit desk for assistance.
