Citizens Memorial Hospital (CMH) in Bolivar, MO is hiring a Care Coordinator to join our Care Coordination Department in the Emergency Department! CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. About the department: Care Coordination provides support to patients by helping with navigating the healthcare system and facilitates care both inside and outside the patient's primary care clinic to improve patient outcomes.
A typical day includes: The Care Coordinator and Referral Specialist play a critical role in supporting the Emergency Department by ensuring patients receive timely followup care, referrals, and scheduling after their ER visit. This position bridges the gap between acute care and outpatient services, reduces unnecessary return visits, and improves patient outcomes by coordinating nextstep care with primary care providers, specialists, and community resources.
This role is essential to maintaining a smooth patient flow, reducing provider administrative burden, and ensuring patients leave the ER with clear, actionable plans for continued care.
Care Coordination and Referral Management
- Review ER discharge plans and identify required followup appointments, diagnostic testing, or specialty referrals.
- Ensure patients understand their next steps, including timelines, instructions, and care expectations.
- Coordinate with case management, social work, and nursing staff to address barriers to care (transportation, insurance, access issues).
- Support transitions of care for highrisk patients, including those with chronic conditions, frequent ER utilization, or complex medical needs.
- Process referrals to primary care, specialty clinics, behavioral health, and community partners.
- Verify insurance requirements, obtain prior authorizations when needed, and ensure referrals meet payer guidelines.
- Communicate with outpatient clinics to secure timely appointment availability.
Scheduling & FollowUp
- Schedule followup appointments directly for patients prior to discharge whenever possible.
- Coordinate imaging, lab work, and specialty consults based on provider orders.
- Support ER providers by managing outbound scheduling tasks so they can focus on clinical care.
Communication & Collaboration
- Serve as a liaison between the ER, primary care clinics, specialty departments, and external providers.
- Maintain realtime communication with ER physicians, APPs, and nurses.
- Build strong relationships with outpatient clinics to streamline scheduling pathways.
Documentation & Compliance
- Document all coordination activities in the EMR accurately and promptly.
- Ensure compliance with HIPAA, EMTALA, and organizational policies.
- Maintain logs and tracking tools to support quality improvement and reporting.
- Participate in audits, performance reviews, and process improvement initiatives.
Job Requirements:
- Education: HS diploma
- Experience: Preferred 3 years clinical or other healthcare experience
- Skills: Customer Service, medical terminology
- Licensure/Certification: N/A
- Physical: ability to work at a computer 90% of the day and stand for the remaining 10%
