Mercy Flights is Hiring a Mobile Integrated Healthcare Community Health Worker (MIH-CHW)!

Our Mobile Integrated Healthcare (MIH) program is designed to bridge gaps in care, reduce barriers to services, and improve long-term health outcomes through proactive, patient-centered support. We are seeking compassionate, motivated, and adaptable Community Health Workers (CHWs) who are passionate about serving vulnerable populations and helping transform healthcare delivery in our community.

The Role

As a Mobile Integrated Healthcare Community Health Worker (MIH-CHW), you’ll work alongside multidisciplinary healthcare teams to provide proactive outreach, post-hospital follow-up care, patient education, crisis intervention support, and care coordination services in homes and community settings.

In this role, you will:

  • Conduct in-home patient assessments and follow-up visits
  • Assist with chronic disease management and preventative health education
  • Support patients with care coordination, transportation, referrals, and access to community resources
  • Respond to behavioral health concerns, psychiatric crises, and urgent patient needs with compassion and professionalism
  • Provide support for patients navigating mental health or substance abuse recovery
  • Coordinate with physicians, hospitals, nurse case managers, and community agencies to ensure continuity of care
  • Perform home safety evaluations, basic vital checks, wound care, and socioeconomic assessments
  • Educate patients and families on disease prevention, self-care, medication adherence, and chronic condition management
  • Deliver care in non-traditional settings focused on long-term wellness and reducing unnecessary emergency utilization
  • Maintain accurate electronic documentation and patient care records in compliance with state and federal requirements

Work Environment & Schedule

  • Work performed in clinical, office, in-home, and community-based settings

Required Qualifications

Education

High School Diploma or equivalent required

Associate degree in healthcare, human services, or related field preferred

Experience

  • Minimum of one (1) year of healthcare and/or social services experience and/or education required
  • Experience in behavioral health, substance abuse recovery, or mental health services preferred
  • Licenses & Certifications
  • Community Health Worker (CHW) certification required within one (1) year of hire
  • Valid Oregon Driver’s License required
  • CPR certification required

Preferred Qualifications

  • Bilingual/Bicultural skills, particularly Spanish proficiency
  • Experience with Mobile Integrated Healthcare or community health programs
  • Knowledge of chronic disease management, preventative medicine, and community health resources
  • Familiarity with behavioral health and crisis intervention support
