About This Position
Mercy Flights is accepting applications for a full-time MIH Community Healthcare Worker.
Under indirect supervision, works closely with the MIH team, primary care teams, and social services agencies to provide short-term care coordination and connection to resources and support to program clients to improve their health and general well-being through education and coordination of care and services. Works in both clinical and community-based settings, including client's homes. May act as a peer support worker, where applicable, if self-disclosed as a current or former consumer of mental health or substance abuse services with at least two years of mental health or substance abuse recovery.
Position Information:
- Full-time schedule that promotes work-life balance and weekends off. The CHW position features a rotating schedule with 10.5-hour shifts from 8:30 AM to 7:00 PM, alternating between on and off days over a two-week period, totaling 84 hours.
- Daily overtime if shift exceeds 12 hours.

Essential Duties and Responsibilities:
- Provide quality patient care for clinical operating guidelines and policies, protocols, and accepted standards of care.
- Maintains comprehensive electronic client files, which include client notes, the release of information, assessments and other medical documents acquired on behalf of the client.
- Complete patient care reports promptly per Oregon State law company protocols and guidelines.
- Conduct proactive and post-hospital visits
- Provide in-home assessments and health education
- Support patients with chronic conditions and navigate social determinants of health
- Offer peer support in mental health or substance abuse recovery
- Collaborate closely with healthcare teams and community partners
- Work in both clinical and non-traditional care environments
- Perform basic clinical care such as vital checks and wound care
- Educate patients and families on self-care and disease prevention
This is a field-based position involving local travel throughout our service area. A full-time rotating schedule with 10.5-hour shifts offers work-life balance and weekends off.
- Build and maintain positive working relationships with clients, providers, nurse case managers, agency representatives, supervisors, and office staff from diverse cultural and socio-economic backgrounds.
Qualification Requirements:
- A High School Diploma or equivalent is required.
- Oregon Driver’s License
- CPR certification
- An associate degree in health care or human services is preferred. A combination of education and experience will be considered
- Any combination of one (1) year of health care/social services experience and/or education is required; two (2) years is preferred
- Experience in the substance abuse and/or mental health field is preferred
- Community Health Worker certificate (required to obtain within one year of start date)
