Summary of Position

This position provides Care Management with reliable and dependable weekend staff coverage (Fri-Sun, 12-hour shifts 7a-7:30p). This position is responsible for providing care coordination, comprehensive discharge planning, support for utilization review/management, readmission avoidance action plan strategies, and care transitions coordination under the guidance of the Director and Manager of Care Management. The Case Manager- Weekend Team Lead participates in a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the options and services for a patient’s individual health needs under the Scope of Service for the department. Responsible for daily operations to coordinate high-quality cost-effective care at Saratoga Hospital. Responsibilities include meeting all compliance and regulatory agency guidelines.

The primary focus of the Case Manager- Weekend Team Lead is to provide support to Case Managers in both the ED as week as inpatient units for discharge planning needs and utilization review of observation and inpatient criteria based on National MCG Criteria. The Case Manager- Weekend Team Lead Is a critical thinker and problem solver to resolve challenges and barriers to achieve care management goals and safe disposition across the care continuum. As Case Manager Weekend Team Lead, this role serves as a resource and supports weekend Care Management staff and follows up with hand-off communications. Follows through with the Director and Manager for any issues as indicated. Possesses strong interpersonal skills to work with care management team, interdisciplinary team members and community partners to reach quality patient outcomes.

šŸ” What You’ll Do

    • Coordinates discharge planning activities to support effective discharge timely and efficiently through collaboration with providers and multi-disciplinary team.
    • On Friday’s rounds with the Case Managers and Social Workers to identify cases requiring weekend follow up.
    • Works with Manager of Care Management to develop and assesses weekend care management assignment based on weekend staffing and census.
    • Provides support to Weekend ED and IP CM’s
    • Works to minimize discharge delays and achieve appropriate dispositions to the community.
    • Refers high risk patients to community resources for complex and dual planning needs as indicated.
    • Knowledgeable of Social Determinants of Health needs and resources to meet those needs.
    • Has ability to facilitate appropriate transfers on the weekends as indicated.
    • Document the care plan and required data in electronic medical record and follows the Care Management Plan Policy.
    • Initiates timely community resource referrals, in alignment with patients’ benefits, within electronic medical record as needed based on patient choice and post-hospitalization needs for discharge and transfer.
    • Coordinates interdisciplinary collaboration to achieve patient safety and a safe discharge plan.
    • Maintains a working knowledge of the resources available in the community and requirements of government payers and managed care organizations.
    • Provides appropriate linkages, referral coordination, and follow-up.
    • Collaborate with community liaisons and health care partners to develop, plan, and facilitate optimal discharge plans.
    • Advocates for the patient's and family's needs.
    • Arrange patient care conferences as needed to facilitate complex discharge planning, improve communications, and achieve quality patient outcomes.
    • Escalates identified barriers or concerns regarding any aspect of their role to the Manager or Director of Care Management.
    • Issues ā€œImportant Medicare Messageā€ (IM) as required 24-48 hours prior to discharges of Medicare and/or Managed Medicare patients.
    • Serves as a resource to physicians, patients/families regarding insurance coverage/reimbursement. Adheres to established guidelines for working with insurance case managers and utilization coordinator.

RN or LMSW. For RN-BSN preferred.

Prior Case Management and Utilization Management experience required.

Minimum of 3 years acute care management hospital experience

Knowledgeable of New York State and Federal Entitlement Programs and regulatory requirements.
