Job Summary: The Veteran Services Program Liaison provides strategic leadership for the design, implementation, growth, and long-term sustainability of NorthStar Care Community's veteran-focused initiatives across Michigan. This role serves as the organization's primary expert and ambassador for veteran-centered hospice care, leading statewide education, cultivating strategic partnerships, advancing community engagement, and ensuring exceptional experiences for veterans and their families. Through collaboration with operational leaders, community organizations, and veteran advocates, this position drives innovation, expands outreach, and strengthens NorthStar's reputation as a leader in veteran end-of-life care.
Essential Functions:
1. Develops and executes the organization's long-term strategic vision for veteran services, ensuring alignment with organizational priorities and national best practices.
2. Leads the implementation and integration of the veteran training program across interdisciplinary teams throughout the state.
3. Creates and maintains educational materials, learning resources, and program tools that support effective training delivery and ongoing program success.
4. Identifies opportunities to expand veteran programming, pursue recognition opportunities, grant funding, and collaborative partnerships that strengthen organizational impact.
5. Partners with leadership to establish annual goals, key performance indicators, and strategic initiatives related to veteran engagement and recognition.
6. Plans, coordinates, and facilitates engaging workshops, webinars, in-services, and in-person educational sessions for hospice and palliative care teams.
7. Develops dashboards and regularly reports program outcomes, education participation, community engagement, and organizational impact to senior leadership.
8. Builds and supports an internal network of staff champions to promote veteran-centered care initiatives and sustain program engagement.
9. Leads multidisciplinary workgroups and influences organizational initiatives without direct supervisory responsibility.
10. Serves as the organization’s subject matter expert on veteran care, military culture, and cultural competency related to veterans and their families.
11. Travels throughout Michigan to support regional teams, cultivate community partnerships, represent NorthStar Care Community at veteran events, and ensure consistent implementation of veteran-centered initiatives.
12. Actively participates in interdisciplinary team meetings, organizational initiatives, community events, educational opportunities, and professional development activities to support collaboration and continuous growth.
13. Represents and promotes NorthStar Care Community’s mission, values, and veteran-focused initiatives through local, regional, and statewide veteran organizations, conferences, advisory councils, and community events.
14. Ensures compliance with all organizational policies, procedures, protocols, regulatory standards, legal requirements, and the organizational Code of Conduct.
15. Adheres to organizational Code of Conduct, policies, procedures, protocols and processes and all regulatory and legal requirements.
16. Adheres to the NorthStar standards to care for every person, every time, 100% of the time.
