Pharmacy Technician Population Health

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Pharmacy Technician Population Health based in the United States.

This role sits at the intersection of pharmacy operations and population health, focused on improving medication adherence and patient outcomes across diverse populations. You will support pharmacists and care teams through proactive outreach, refill coordination, and identification of barriers that impact access to therapy. Working in a remote healthcare environment, you will contribute to population-based initiatives such as adherence campaigns and quality measure improvements, including Medicare Stars programs. The role requires strong attention to detail, excellent communication skills, and the ability to follow structured workflows while supporting patient-centered care. You will play an important part in ensuring continuity of treatment and enhancing the overall patient experience through timely intervention and coordination. This is a highly collaborative position where your work directly contributes to improved health outcomes and system-wide quality performance.

Accountabilities
  • Conduct inbound and outbound patient outreach to support medication adherence, including refill reminders and follow-ups on gaps in therapy.
  • Identify, document, and escalate barriers to adherence such as cost, access, side effects, or understanding of therapy.
  • Coordinate prescription refills, pharmacy transfers, and prior authorization follow-ups in collaboration with pharmacists and care teams.
  • Maintain accurate and timely documentation in pharmacy and population health systems in compliance with policies and regulations.
  • Support adherence and quality improvement campaigns aligned with organizational and Medicare Stars measures.
  • Collaborate with pharmacists, providers, and population health teams to close care gaps and improve patient outcomes.
  • Provide high-quality customer service via phone and other communication channels in accordance with approved scripts and workflows.
  • Assist in medication preparation workflows, calculations, and pharmacy technology processes as appropriate to scope and state regulations.
  • Participate in departmental initiatives, meetings, and continuous improvement efforts to enhance workflows and patient satisfaction.
  • Escalate clinical concerns and safety issues to pharmacists as appropriate while maintaining compliance with privacy and regulatory standards.
Requirements
  • Active Pharmacy Technician license in Utah, Colorado, Montana, or Nevada (required).
  • High school diploma or equivalent required.
  • Current pharmacy technician certification and compliance with applicable state and federal regulations.
  • 1–5 years of pharmacy technician experience preferred, ideally in a health system or retail environment.
  • Experience with medication adherence programs or Medicare Stars quality measures is strongly preferred.
  • Strong communication skills, especially for phone-based patient interactions.
  • Excellent attention to detail and ability to follow structured clinical and operational workflows.
  • Strong organizational skills and ability to thrive in a fast-paced, remote healthcare environment.
  • Proficiency with computer systems, pharmacy platforms, and Microsoft Office or Google Workspace tools.
  • Ability to collaborate effectively within interdisciplinary healthcare teams.
  • Strong problem-solving skills with the ability to identify and escalate clinical or operational issues appropriately.
