Position Summary

The Financial Navigator helps uninsured and underinsured patients access affordable care. The position primarily manages Sliding Fee Discount Program applications and renewals while screening patients for possible Medicaid, Marketplace, Medicare, or other coverage options. The Financial Navigator provides patient education, application support, referrals, and follow-up while ensuring accurate documentation and compliance with organizational requirements.

Principal Duties and Responsibilities

  • Screen uninsured patients for potential Medicaid, Marketplace, Medicare, or other coverage options.
  • Explain the Sliding Fee Discount Program, eligibility requirements, available discounts, and patient responsibilities.
  • Help patients complete sliding fee applications and annual renewals.
  • Review income and household documentation, determine eligibility, and document approvals and expiration dates.
  • Follow up with patients regarding incomplete applications or upcoming renewals.
  • Track application volume, completion, renewals, and coverage-navigation outcomes.
  • Assist patients with gathering required information and connect them with qualified enrollment partners when appropriate.
  • Assist patients with completion of UNCFA and Presumptive Medicaid application
  • Conduct outreach by phone, text, patient portal, mail, or in person.
  • Accurately document applications, outreach, referrals, and outcomes in the electronic health record.
  • Maintain patient confidentiality and follow HIPAA, HRSA, and organizational requirements.
  • Work with registration, scheduling, billing, outreach, and community partners to address financial barriers to care.

Requirements:

Required Skills or Abilities

  1. Bilingual (Spanish) requirement
  2. Computer literacy in internet use and Windows environment, including Outlook, Word, and Excel.
  3. Ability to communicate clearly and succinctly.
  4. Must be able to work with changing priorities.
  5. Requires excellent organizational, problem solving and critical thinking skills.
  6. Requires the ability to maintain confidentiality.
  7. Must be able to function as part of a team.
  8. Ability to cultivate and develop inclusive and equitable working relationships with co-workers and community members.
  9. Ability to serve as an advocate for individuals of all ethnicities, genders, ages, and backgrounds.

Required Knowledge, Experience, or Licensure/Registration

  1. High School education or equivalent
  2. Ability to work onsite Mondays through Fridays during the hours of 7:45 am and 5 pm.
  3. Ability to read, write, speak, and comprehend English fluently as needed to perform essential job functions, including patient communication, documentation, and safety-related instructions.

Physical requirements of the Job

With or without reasonable accommodation, this position requires the ability to:

  1. Sit or stand, often for prolonged periods
  2. Carry or lift objects up to 20 pounds

Health and Safety Requirements

Up-to-date immunizations as required for the role, subject to medical or religious exemption requests in accordance with applicable law and NHC policy.

This is a full-time, non-exempt position.
