Job Description
The RN Wellness Coach reports to the Practice Manager and provides professional nursing care, preventive health coaching, chronic disease management, and wellness program development in support of physician-directed medical care within Beacon Health System Paragon Worksite Clinics.
This role blends direct patient care, health coaching, wellness education, occupational health support, and strategic employer wellness programming to improve individual and population health outcomes. The RN Wellness Coach supports onsite/nearsite employer clinics by delivering high-quality nursing care, one-on-one and group wellness coaching, disease prevention strategies, chronic condition management, and client-specific wellness initiatives.
The RN Wellness Coach works collaboratively with physicians, organizational leaders, employer clients, HR teams, and patients to develop, implement, and evaluate innovative health promotion programs focused on improving employee health, reducing chronic disease burden, increasing workplace wellness engagement, and enhancing overall clinic value.

Clinical Nursing Care & Patient Support
- Provide professional nursing care for patients following established clinical standards and physician direction.
- Prepare patients for examinations, treatments, and procedures.
- Administer medications, injections, and treatments as prescribed.
- Assist physicians in examinations, procedures, and patient management.
- Collect patient histories, vital signs, biometric data, and specimens.
- Perform laboratory duties including specimen collection, tracking, and result communication.
- Conduct venipuncture, EKGs, and other clinical nursing procedures as required.
- Educate patients and families on treatment plans, medications, disease prevention, and wellness strategies.
- Coordinate referrals, pre-certifications, and continuity of care services.
- Maintain accurate and timely EMR documentation.
Wellness Coaching & Chronic Disease Management
- Coach patients individually and in small groups to achieve health and wellness goals related to:
- Weight management

- General lifestyle improvement
- Develop personalized wellness plans based on patient goals, health risks, and physician recommendations.
- Monitor patient progress and adjust coaching strategies accordingly.

- Tobacco dependence
- Provide guideline-based health counseling and evidence-based behavior modification support.

Program Development & Population Health
- Develop and implement comprehensive wellness programs for employer populations including:
- Weight management
- Diabetes prevention and education
- Tobacco cessation
- Stress reduction
- Preventive screenings
- Healthy lifestyle initiatives
- Collaborate with employer clients, HR leaders, and organizational stakeholders to integrate wellness into workplace culture.
- Design chronic disease education programs and population-specific health interventions.
- Lead wellness classes, seminars, workshops, and off-site educational events.
- Analyze biometric, lab, and health risk assessment data to customize wellness initiatives.
- Track participation, outcomes, engagement metrics, and program ROI.
- Support employer-specific strategic health initiatives that improve workforce wellness and reduce healthcare risks.
Administrative, Scheduling & Documentation Support
- Manage personal coaching and wellness clinic schedules.
- Conduct appointment preparation and follow-up patient calls.
- Document all coaching sessions, assessments, attendance, and outcomes within EMR.
- Track patient participation, graduation, and follow-up for wellness programs.
- Support clinic workflow efficiency through timely patient communication and scheduling coordination.
- Respond to patient, pharmacy, and clinical inquiries professionally.
- Support front-office and back-office operational needs as required.
Communication & Collaboration
- Serve as a wellness and clinical liaison between:
- Physicians
- Practice leadership
- Employer clients
- HR departments
- Clinical staff
- Patients
- Communicate health trends, risks, and opportunities to organizational leadership.
- Promote patient engagement and employer wellness participation.
- Build strong relationships with clients and stakeholders to advance wellness culture.
Organizational Responsibilities
- Attend and participate in department meetings.
- Complete all mandatory education, competencies, certifications, and employee health requirements.
- Maintain active RN licensure and required certifications.
- Adhere to HIPAA, regulatory, safety, and organizational policies.

ORGANIZATIONAL RESPONSIBILITIES
Associate complies with the following organizational requirements:
- Attends and participates in department meetings and is accountable for all information shared.
- Completes mandatory education, annual competencies, and department specific education within established timeframes.
- Completes annual employee health requirements within established timeframes.
- Maintains license/certification, registration in good standing throughout fiscal year.
- Direct patient care providers are required to maintain current BCLS (CPR), and other certifications as required by position/department.
- Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.
- Adheres to regulatory agency requirements, survey process and compliance.
- Complies with established organization and department policies.
- Available to work overtime in addition to working additional or other shifts and schedules when required.
Education and Experience
- Graduate of an accredited nursing program.
- Current Registered Nurse (RN) license in Indiana required.
- Minimum of one year of related nursing experience required.
- Medical office, occupational health, wellness coaching, or chronic disease management experience preferred.
- Wellness Coach Certification preferred.
- Experience in employer wellness, population health, or worksite clinic settings preferred.

Physical Demands
- Ability to stand, walk, and move for prolonged periods.
- Lift up to 50 pounds occasionally.
- Position or assist patients as required.
- Perform CPR and emergency interventions.
