Job Posting: Intake and Placement Manager

Location: Rochester, NY

Department: Centralized Intake

Employment Type: Full-Time

Schedule: M-F, 40 hours

Salary: $62,353.00-65,000/yr.

Essential Duties and Responsibilities

  • In collaboration with program leadership, develop, implement, and monitor policies and procedures for client outreach, engagement and centralized intake processes.
  • Serves as an active member of the management team in planning, problem solving and ongoing communication.
  • Provides supervision, guidance to intake and outreach/engagement staff, and executes the duties common to all supervisory positions to include but not limited to:

a. Regularly meets with direct reports.

b. Completes annual performance appraisals.

c. Regularly monitor performance and provide appropriate feedback.

  • In coordination with Human Resources, participates in hiring, onboarding, retention, coaching, progressive discipline, and separation activities of employees.
  • Assists in budget development and is responsible for monitoring budgeted expenses for outreach and intake activities.
  • Facilitate in a variety of administrative projects relevant to outreach, engagement and intake ensuring projects are within scope, timeline, budget limits and forecasts projects related.
  • In coordination with marketing and program development, assist in the design of outreach and engagement material, marketing and placement plans, using messaging tailored to various audiences including clients, families, providers, and community partners
  • In coordination with IT and data system staff, leads efforts for agency wide data system for client demographics, service inventory and Social Determinants of Health.
  • In coordination with IT and other community partners, serves as lead to implement and fully utilize referral systems.
  • Develops, implements, monitors, and evaluates agency outreach and engagement plan.
  • Coordinates and schedules staff for agency and program related outreach, engagement for community events, fairs, and network opportunities
  • Serves as a role model for staff and demonstrates CCFCS values in outreach, engagement, and intake activities.
  • In coordination with peer services, ensure outreach, engagement and intake are utilizing best practices and trauma informed approaches in all services.
  • Develop, implement, and monitor internal information networks to improve staff education and knowledge of CCFCS spectrum of services.
  • Represents CCFCS on community boards, networks, and collaborations and provide communication and education on CCFCS services and referral processes.
  • Actively pursues development of professional competencies related to job performance through reading, supervision, in-service training and attendance at conferences and workshops.
  • Participates in relevant Agency meetings and/or trainings
  • Supports and fosters the Agency's Mission, Vision, Values and Strategic Plan.
  • Other duties as assigned.

Requirements:

Qualifications

Education Bachelor’s Degree in health and human services or related field. Equivalent Combination of education and experience will be considered.

Credentials Valid and clean NYS Driver License.

Experience Three years’ experience with intake or assessment activities in health and human services. Three-years supervisory experience.
