SUMMARY
The Coordinator – Patient Safety & Quality supports the Patient Safety Manager in the planning, implementation, and evaluation of a comprehensive Patient Safety Program. This role focuses on the review, analysis, and follow-up of safety events; participation in Root Cause Analyses (RCAs) and other safety investigations; and supporting compliance with CMS patient safety structural measures, DNV patient safety standards, and other applicable regulatory and accreditation requirements. The Patient Safety Coordinator promotes a culture of safety, transparency, and continuous learning across the organization.
RESPONSIBILITIES
- Supports the Patient Safety Manager in the day-to-day operations of the Patient Safety Program.
- Reviews, triages, and coordinates follow-up of reported safety events in the electronic reporting system.
- Identifies trends, near misses, and potential high-risk events requiring escalation or further review. Recommends improvement opportunities of priority areas.
- Participates in and supports Root Cause Analyses (RCAs), Failure Mode and Effects Analyses (FMEAs), After Action Reviews (AARs), and other safety investigations.
- Assists with data collection, analysis, meeting coordination, documentation, and action plan tracking related to RCAs and safety reviews.
- Supports implementation and monitoring of corrective actions to ensure effectiveness and sustainability.
- Develops safety reports, dashboards, and presentations for leadership, committees, and governance.
- Participates in patient safety committee, safety huddles, system falls committee, and interdisciplinary meetings.
- Collaborates with Quality, Risk Management, Infection Prevention, Nursing, Medical Staff, and other departments to address safety concerns.
- Supports organizational compliance with CMS patient safety structural measures and DNV patient safety standards.
- Assists with policy development, education, and communication related to patient safety and just culture principles.
- Promotes a culture of safety through staff engagement, education, and recognition programs.
- Performs other duties as assigned
QUALIFICATIONS
- Current RN Licensure for South Carolina.
- Bachelor’s degree in Nursing preferred.
- Minimum of three (3) years of clinical, quality, risk management, or patient safety experience required. Experience with participating in RCAs or safety investigations preferred.
- Knowledge of patient safety principles, quality improvement methodologies, event reporting systems, and regulatory patient safety requirements including CMS and DNV standards preferred.
- Ability to effectively and professionally communicate to cross-functional groups and teams, including organizational leaders and physicians.
