Job Description
POSITION SUMMARY/RESPONSIBILITIES
Provides leadership in the provision and coordination of patient care for patients served by the Supportive Care Services (Palliative Care) program. Formulates and guides the plan of care in collaboration with physicians, advanced practice providers, social workers, chaplains, case management, and other members of the interdisciplinary healthcare team. Performs comprehensive nursing assessments, supports symptom management, coordinates care across inpatient and outpatient settings, and promotes continuity of care for patients with serious illness. Serves as a clinical resource for nursing staff, mentors colleagues within the department, and supports quality improvement initiatives while providing compassionate, patient-centered palliative nursing care.
EDUCATION/EXPERIENCE
Must meet career ladder Staff Nurse II criteria for the assigned work area. Two (2) years or more of nursing experience required. Experience in acute care, critical care, oncology, geriatrics, hospice, supportive care, or related specialty is preferred. Experience in symptom management, interdisciplinary care coordination, and patient assessment is strongly preferred. Experience with quality improvement activities, chart review, or clinical documentation is preferred. BSN highly preferred.
LICENSURE
Must possess a current RN license from the Board of Nurse Examiners of the State of Texas. Must have a current AHA BLS Healthcare Provider or AHA BLS Instructor Provider card.
