Team Lead - Case Management in Westminster, Maryland at LifeBridge Health

The Case Management Team Lead supports daily operations of the care management department while serving as a mentor, resource, and first-line support for case management staff. This leadership role combines direct case management responsibilities with oversight of workflow, staffing, productivity, discharge planning compliance, and staff development.

Key responsibilities include:

• Supervising and guiding daily case management operations to ensure department standards are met
• Mentoring and educating staff on care management processes, discharge planning standards, and documentation practices
• Monitoring staff productivity, quality outcomes, and EMR documentation accuracy
• Assisting with staffing schedules, assignments, and coverage coordination
• Serving as the first point of contact for operational and workflow issues within the department
• Supporting hiring, onboarding, orientation, and development of new team members
• Participating in employee performance evaluations and disciplinary processes when needed
• Promoting positive patient experiences and relationship-focused care
• Demonstrating strong communication, organizational, leadership, and resource management skills
• Working independently while collaborating closely with the Care Management Manager and interdisciplinary teams

The role requires clinical knowledge, leadership ability, problem-solving skills, and expertise in discharge planning, utilization management, and interdisciplinary care coordination.

Experience

• 3–5 years of related experience required

Education

• Nursing degree or Master of Social Work (MSW) required

Licensure/Certifications

• Current Maryland RN license or Social Worker license required
• Stronhlu Preferred: Case Manager Certification (ACM or CCM)
• ACM/CCM certification required within 3 years of hire if not already held
