Job Description

Under the supervision of the leadership team, the Referral Coordinator manages patient referrals for program services recommended by their provider. This position will primarily focus on ensuring patient referrals are routed to the appropriate programs and disciplines.

  • Works with electronic platforms and documents to identify and route referrals to Ambulatory Care Management programs.
  • Retrieve program referrals from multiple sources (EMR, Email, Phone, Risk Reports etc. )
  • Tracks referral status and performs patient outreach as necessary.
  • Troubleshoots all program problems relating to the referral processes and escalates to appropriate leaders when necessary.
  • Assists leadership in ensuring program referral KPIs are accurately tracked and reported regularly.
  • Ensures program KPIs are appropriately tracked as part of the program dashboard.
  • Engages analytics and reporting teams as necessary to develop reporting dashboards, Regularly reports KPIs to leadership.
  • Monitors reports for any inaccuracy or integrity issues, and promptly engages stakeholders to resolve issues.
  • Ensures the highest quality of service and care to those we serve. Committed to satisfying internal and external customers and exceeding their expectations.
  • Builds and fosters trust and credibility.
  • Maintains awareness of quality indicators and patient satisfaction. Participates in quality monitoring and reporting as requested.

Experience Required: 1-3 years relevant experience

Education Preferred: High School / GED
