Note: The compensation range noted above represents the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range.

Holyoke Medical Center is looking to hire a Nurse Practitioner or Physician Assistant to work at the School-Based Health Center located at Holyoke High School North Campus. This individual will provide a full array of health services as a primary care provider on the school-based health center team for students seeking health services within the school-based clinic. This position is grant funded through a Massachusetts Department of Public Health Grant. This position is a ten (10) month position that follows the school calendar year.

Overview of responsibilities include but are not limited to:

  • Conduct comprehensive student health assessments, including developmental/health histories, physical exams, and basic laboratory testing.
  • Assess, diagnose, and treat adolescent health needs, recognizing normal and abnormal growth and development.
  • Provide immunizations, preventive care, wellness services, counseling, and anticipatory guidance on nutrition, growth, and development.
  • Evaluate illnesses, injuries, safety/health risks, and developmental concerns; provide treatment within established protocols and make appropriate referrals.
  • Develop individualized medical care plans and provide timely follow-up and case management in collaboration with the clinic team and outside providers.
  • Maintain accurate, timely, and systematic clinical documentation while ensuring patient rights, privacy, and confidentiality.
  • Communicate effectively with students, families, school personnel, healthcare providers, and community agencies from diverse cultural backgrounds.
  • Collaborate with school nurses, counselors, educators, administrators, and clinic staff to address student health needs and participate in school health activities.
  • Work across multiple Teen Clinic sites, organize responsibilities efficiently, communicate with supervisors, and contribute to quality patient care and positive relationships.
  • Maintain professional licensure and competency through continuing education and professional development, while keeping knowledge and clinical practices current.

Qualifications:

Education: Graduate of a formal Nurse Practitioner's program or Graduate of a Certified Physician Assistant program

Licenses/Registrations: Licensure to practice as a Nurse Practitioner in the Commonwealth of Mass or Current Massachusetts Physician Assistant License

Experience: One year in a professional Nurse Practitioner or Physician Assistant role with experience in school-based health preferred.

Special Aptitudes & Activities: Bilingual (Spanish/English) ability aplus. The ideal candidate demonstrates strong communication, organizational, and multitasking skills, with the ability to effectively collaborate with patients and healthcare staff. They must be able to perform comprehensive and focused patient assessments, interpret diagnostic data, develop treatment plans, and exercise sound clinical judgment when consultation or referral is needed. The role also requires attention to detail, proficiency with computer systems and medical equipment, and the ability to provide high-quality care to adult and geriatric patients in a culturally diverse environment.
