Job Description

The Home Care Scheduler / Coordinator is responsible for planning and scheduling patient visitations and tasks for the home health / personal care aide staff. Major responsibilities include maintaining daily staff schedules, coordinating services with contracted agencies, constant communication with staff regarding all job related issues, preparing reports as necessary. Qualifying all requests for proper insurance information and authorization where applicable.

Normal Schedule: Monday - Friday 8:30-4:30pm with on-call rotation required

Duties and Responsibilities:

  • Collaborates with staff and supervisor to provide continuously updated information regarding patient needs and patient care as well as the needs of the clients.
  • Handles and processes requests for home care services/referrals..
  • Assigns personnel as directed by the Director of Patient Services/designee using internal systems for scheduling and tracking.
  • Ongoing communication with staff regarding job assignments, requirements and other job related issues.
  • Creates and maintains all pertinent patient, personnel and client files.
  • Contact patients, insurance providers, clients and others to ensure compliance with current business related policies and procedures.
  • Verify timecards and identify any variances in actual versus time scheduled for payroll and billing.
  • Prepares reports as necessary.
  • Participates in quality assurance, employee recognition activities and corporate representation as assigned.
  • Present the organization in a pleasant and professional manner and maintain strict confidentiality of information at all times.
  • On-call responsibilities and other duties as assigned.

Qualifications:

  • Demonstrates a sympathetic attitude toward those in need of care.
  • Strong verbal and written communication skills.
  • Associates degree (or higher) or relevant work experience in a similar setting.
  • Demonstrates excellent telephone manner.
  • Strong organizational skills and the ability to work under pressure.
  • Computer skills including familiarity with Microsoft Windows and Office products.
  • Willingness to accept on-call assignments when necessary.

The pay range displayed on this job posting reflects the anticipated range for this position. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set, and education. This is not inclusive of the value of the benefits package, which includes, among other benefits, healthcare/dental/vision, PTO, and retirement.
