Whether you are transitioning toward retirement, seeking a reset from high-stress hospital environments, or simply needing a predictable schedule for your family, this Office-Based BHH Nurse Care Manager role is your perfect landing spot. • Consultative Care: Guide clients transitioning out of hospitals or emergency rooms to ensure safe, successful discharges and complete accurate medication reconciliations.
  • Safety Advisory: Provide clinical guidance during crisis situations and advise AWS management on health, safety, and infectious disease protocols.

About Alternative Wellness Services (AWS) Exceptional client care begins with how we support our team. AWS is a dynamic, forward-thinking agency that values innovation, collaboration, and recovery-oriented care.
