Job Description
I. The employee in this position will provide support and accompany individuals on a variety of outings, events and activities that support their ISP objectives and goals. Team member will provide safe transportation for individuals to field trips, appointments, school and upon emergency circumstances in St. Mary’s insured vehicles. Team member will also help the individuals develop relationships with members and organizations in the community. They will also assist with medical appointments as scheduled by the appointment nurse.
II. The employee in this position will have a flexible schedule and will be required to work every other weekend, early release or when there is no school (summer break, holiday break, spring break, etc.).
Requirements:RESPONSIBILITIES:
A. Behavior should be professional at all times and consistent with St. Mary’s Home mission, policies, and procedures, including treating the individuals and others with dignity and respect at all times.
B. Provide meaningful interactions with the individuals during the outing to enhance the experience of it while incorporating ISP objectives and goals.
C. Provide safe transportation for the individuals to community outings and medical appointments. During periods when field trips are not scheduled, the employee will be available to assist in other ways needed by the facility.
D. Responsible for assisting the Direct Support Professional who works with the individuals with personal hygiene, lifting, toileting, and dressing appropriately for the weather and type of activities as needed. Assembles supplies that will be needed on the field trip and/or medical appointment (i.e., diapers, sunscreen, umbrellas, meals, snacks, etc.).
E. Communicate to the nurse any concerns about the health status of the individual upon return from the outing or appointment.
F. Report immediately any concerns about the vehicles to the Community Integration Specialist Supervisor and/or maintenance.
G. Reports and/or documents routine and unusual events including completing field trip documentation using CareTracker, daily vehicle visual inspections (i.e. tie down system, lights on the van, first aid kit, van lift, etc.), accident reports, etc. in a concise, understandable, and legible manner.
H. Must attend St. Mary’s mandatory in-services (infection control, OSHA regulations, Relias training, driving training and regulations, confidentiality, behavior plans, etc.).
I. Must Complete basic driving training with an approved instructor from Community Engagement Specialist Team.
· Must pass a competency test including but not limited to:
o 4-point securement training on transport approved wheelchairs and strollers
o Seatbelt training
o Completion of Vehicle log forms
J. Other duties as assigned.

II. PHYSICAL REQUIREMENTS:
1. Must be able to sit, stand and walk for variable lengths of time.
- Must be able to lift up to thirty-five (35) pounds independently, and with or without required equipment (dependent upon the weight of the child), must be able to use proper equipment and procedures to safely lift residents to reposition them, etc.
- While performing the duties of this job, the employee is regularly required to talk and/or hear. Specific vision abilities required by this job include close vision and ability to adjust focus.
III. ADDITIONAL REQUIREMENTS:
- Must wear approved attire for this position (refer to dress code policy).
- Must wear company-issued ID badge at all times.
- Must be 21 years old.
- Must have acceptable driving record based on Fireman’s fund standards. Driving score must be 0 or better.
- Must be able to drive St. Mary’s vehicles and successfully complete the driver training.
- On the Job Training (OJT) - Must complete CPR/First Aid certification and TOVA certification
IV. EDUCATION:
A. Have High School Diploma or GED required, some experience working with individuals with severe disabilities preferred.
B. Use proper grammar and punctuation when writing information about individual behavior, reporting an accident or injury, or other forms of written communication.
