Job Description

This position serves the vulnerable populations to identify resources in the development of an appropriate discharge and plan to address the Social Drivers of Health to prevent readmission. Additionally, this position will work in the community, geo-targeting specific areas that have large populations of uninsured people and will work with community organizations in those areas to leverage resources and reach the most people immediately. This position will build a solid competency on providing information and ensuring their services are available to individuals with diverse levels of education, financial resources, and health literacy. Functions as an extension of Case Management and multi-disciplinary care team to focus on the needs of patients and other citizens of the organization's care area (including, but not limited to Randolph, Montgomery and Moore counties), connecting them with community resources. HS Diploma or GED required. Associate Degree in Human Services or associated field required. Bachelor Degree preferred. College level course work in social work preferred. Training in the medical setting and/or certification as a social worker is preferred. Knowledge of social and healthcare resources within the community. Familiarity with standards for medical and social work practices. Excellent communication skills are required, emphasizing oral and listening abilities. Excellent analytical abilities and capacity to set priorities are required. Knowledge of treatment modalities and alternate healthcare delivery systems is preferred. Minimum of two (2) years’ experience in health care or human services environment. Employment in a healthcare setting working with patients and multidisciplinaryte am. Community outreach experience is also required. Insurance experience preferred.
