1. Proof of Immunisation/Occupational Health: COVID-19,Hepatitis B, MMR, Varicella, BCG and TB - Desirable
2. Passport/Bio-metric card:Acopy of your passport, or the front and back of your bio-metric card
3. Proof of Address:Utility bill from within the last three months or bank statement
4. Work history:The latest version of your CV
5. Enhanced DBS:If you don't have an in-date one, we can sort this out for you. You will need to cover the cost of this
6. Mandatory training certificates:Health, Safety and Welfare, Equality, Diversity and Human Rights, Preventing Radicalisation, Infection Prevention & Control Levels 1 & 2, Information Governance, Fire Safety (Practical), Moving & Handling Levels 1 & 2(Practical), Resuscitation Adults Levels 1, 2 & 3 (Practical), NHS Conflict Resolution (Practical), Safeguarding Adults (Levels 1 & 2), Safeguarding Children (Levels 1 & 2), Resuscitation Paediatric Levels 2 & 3 (Practical)&Resuscitation New-born Levels 2 & 3(Practical).
