About this opportunity
As a Care Manager, you will be responsible for coordinating and managing care services for prospective and existing clients, ensuring a high-quality and consistent client experience. Supporting clients in the Perth Metropolitan area, you will lead the end-to-end client journey including consultation, onboarding, care planning and ongoing reviews, supporting clients to live as independently as possible at home.
Reporting to the Care Manager Team Leader, you will work closely with caregivers, families and internal teams to ensure services are aligned to client needs, compliant with standards and delivered to a high standard.

Key responsibilities
- Manage and coordinate care for prospective and existing clients
- Develop, implement and review individualised care plans aligned to client goals and needs
- Build strong relationships with clients and their families as a key point of contact
- Conduct client onboarding, assessments and regular care reviews
- Collaborate with internal teams to ensure effective caregiver matching
- Maintain accurate client records and documentation
- Ensure compliance with Home Care Package guidelines and Aged Care Quality Standards
- Coordinate with external providers to support holistic care delivery
- Identify opportunities to improve care delivery and client outcomes
About you
You are an organised and client-focused professional who is passionate about delivering high-quality care outcomes. You bring a proactive approach, strong communication skills and the ability to manage multiple priorities in a fast-paced environment.
You'll also have:
- Experience in care management, case management or a similar role
- Understanding of aged care, Home Care Packages or NDIS
- Strong stakeholder and relationship management skills
- High attention to detail and strong administrative capability
- Ability to work both independently and as part of a team
- Relevant qualification in health, social work, nursing or related discipline (preferred)
