McKinney-Vento Homeless Services Case Manager

GENERAL DESCRIPTION: Case management services to be provided to children and their families referred by West Haven Board of Education (BOE) including but not limited to school truancy, food and clothing scarcity, and emotional issues.

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.

EDUCATION AND/OR EXPERIENCE:

  • Bachelor’s Degree preferred in related field, minimum Associates Degree & 1 year experience in Case Management
  • Valid State of Connecticut driver’s license; must be able to use own vehicle, obtain and maintain minimum state insurance coverage and submit coverage documentation
  • Ability to read and write in English and Spanish

ESSENTIAL JOB RESPONSIBILITIES:

Attend weekly meeting with CYS Directors, and as needed meet with WHBOE Pupil Services Grant Manager to provide updates and coordinate duties to avoid duplication of efforts and resources.

Coordinate with school social workers and guidance counselors to establish status of child’s educational needs and resources school is providing.

Establish and maintain contact with parent(s)/guardian(s) on an as-needed, preferably weekly basis; provide educational information on McK-V Grant function and limitations so families understand expectations of program.

Conduct a child and family intake w/parent(s)/guardian(s) establishing current living situation to include employment status, living arrangement, food, clothing, medical needs, and mental health concerns.

Research existing community services to assist families; contact various community resources to establish rapport and provide education re: function of McK-V Grant.

Maintain contact with other WHCH programs that may suit child’s needs and share resource information, help enroll child in program if needed (ex: Head Start, CYS)

Develop a crisis plan for each family; these will vary based on family needs established during intake process.

Maintain up-to-date and accurate records on each child and their families receiving Case Management Services including service/time logs and caseload reports.

Assist families in understanding when a case may be triaged, reviewed, creating an individual service plan, and understanding when the case will be discharged.

Obtain Release of Information (ROI) from parent(s)/guardian(s) as needed to work in conjunction with DCF Case Managers assigned to families and school personnel.

Establish relationship and work with other professional(s) providing family assistance to better understand specific needs of family and gaps in care (including but not limited to: DCF, mental health resources, social workers, teachers, counselors)

Provide referrals to local agencies and mental health care providers for necessary therapeutic support.

Coordinate w/local Universities and service organizations to provide charitable contributions to meet family needs.

Help coordinate and deliver donations to families as needed


Flexible Schedule: Sun-Sat with some hours required Mon-Fri: 20 Hours per week
