Position Highlights

We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our expanding Transitional Care Management Team. This role focuses on caring for high-risk patients recently discharged from the hospital, with the goal of improving outcomes, reducing readmissions, and ensuring a smooth transition back to the community.

Responsibilities

  • Provide Transitional Care Management services to recently discharged patients.
  • Collaborate closely with hospitalists, Emergency Department providers, and Case Management teams at Atrium Health Lake Norman Hospital.
  • Work as part of a multidisciplinary team including Registered Nurses, LPNs, Pharmacists, and Social Workers.
  • Office hours are Monday through Friday.
  • Access additional system-level care management resources and virtual technology.

The Ideal Candidate Will Be:

  • Passionate about providing patient-centered care across a broad spectrum of disease states and chronic conditions.
  • Experienced in hospital-based or Emergency Department settings (preferred).

  • Dedicated to improving care transitions and enhancing patient outcomes through a team-based approach.
