Skilled Nursing Home Physician -MD/ DO - Full Time and Part Time in RUTLAND, Vermont at Community Health Centers of the Rutland Region

POSITION SUMMARY

Community Health Centers of the Rutland Region (CHCRR) is seeking a dedicated and compassionate Physician to provide high-quality, comprehensive care to patients within Skilled Nursing Facilities (SNFs).

The Physician will be responsible for managing complex medical conditions, leading interdisciplinary care teams, and ensuring continuity of care across transitions. This role supports CHCRR’s mission to deliver accessible, patient-centered care while maintaining compliance with regulatory standards and quality initiatives.

ESSENTIAL FUNCTIONS

Clinical Care

• Provide comprehensive medical care to SNF patients with acute and chronic conditions
• Perform initial admission evaluations, develop treatment plans, and oversee discharge planning
• Conduct regular patient rounds and respond promptly to acute and emerging concerns
• Manage medications in collaboration with nursing staff, APPs, pharmacy, lab, and consulting providers
• Coordinate transitions of care across settings to ensure continuity and reduce hospital readmissions
• Complete required clinical documentation in a timely and accurate manner
• Educate patients, families, and caregivers on health conditions and care plans

Care Coordination & Collaboration

• Lead and direct interdisciplinary care teams including nursing, pharmacy, dietary, social work, and care management
• Communicate effectively with patients, families, and care teams regarding treatment plans and goals of care
• Work collaboratively with CHCRR Advanced Practice Providers (APPs) and other clinical staff
• Assist in coordinating transfers and transitions to and from hospitals or other care settings

Quality, Compliance, and Documentation

• Ensure compliance with all federal, state, and facility regulations and standards
• Participate in organizational and facility-level quality improvement initiatives, peer review, and learning cycles
• Complete required documentation related to goals of care, including Advance Directives and COLST forms
• Maintain accurate and timely clinical records to support patient care and billing requirements

Education & Professional Development

• Maintain current licensure, certifications, and continuing medical education (CME)
• Adhere to all professional and ethical standards in the practice of medicine

Leadership Responsibilities

• Provide guidance, clinical protocols, and mentorship to the Community Health APP colleagues and nursing staff.
• Practice antibiotic stewardship, including participation in related educational activity.
• Support and assist with federal and state regulatory surveys.
• Participate in periodic (monthly) quality and safety of care metric review.
