Description:

Job Summary: To provide services in accordance with the policies and procedures as of the Home Care Department, under the direction of a licensed Physician and/or a Registered Professional Nurse.

Demonstrates Competency in the Following Areas:

· Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical condition.

· Demonstrates ability to perform a head-to-toe assessment on all patients and reassessments, as per policy, including pediatric, geriatric and the general patient population.

· Performs patient care responsibilities considering needs specific to the standard of care for the patient’s age.

· Communicates appropriately and clearly to Case Manager, physicians and co-workers, implements emergency actions per department policies.

· Treats patients and their families with respect and dignity.

· Interacts professionally with patient and family and involves them in patient teaching and plan or care.

· Assists nurse manager in the development and implementation of Performance Improvement activities to improve patient outcome and work environment.

· Identifies and addresses psychosocial needs of patients and family.

· Demonstrates ability to be flexible and organized; demonstrates ability to function well under stressful situations.

· Provides direct patient care as assigned by the Case Manager or Director.

· Observes, records, and reports to the R. N. the general physical, mental, and emotional condition of the patient.

· Assists patients with activities of daily living and promotes appropriate self-care to encourage independence.

· Assists in the teaching programs initiated by the R.N. and evaluate the effectiveness of nursing interventions.

· Correlates nursing care plan with medical goals and discharge plan.

· Maintains strict confidentiality of patient information within and without the department.

· Demonstrates ability to provide dressing changes using sterile techniques, assesses and reports the wound status to the physician or R.N.

· Demonstrates ability to provide colostomy care and teaching and documents in patient’s chart.

· Demonstrates ability to provide Foley catheter care: placement, changes, and teaching to Pt/PCG and documents in patient’s charts.

· Demonstrates ability to provide enteral feeding tube care: assesses function, and teaching to Pt/PCG and documents in patient’s chart.

· Demonstrates ability to provide FSBS’s and instructions to PT/PCG on use of the glucose monitoring device and documents progress.

· Is certified in I.V. therapy and is capable of performing venipunctures, caring for I.V. sites and ongoing infusion therapy under the supervision of the R.N.

Requirements:

Professional Requirements:

· Completes annual educational requirements. Completes in-services and staff meetings and returns in a timely fashion.

· Maintains regulatory requirements.

· Reports to work on time and as schedules; consistently completes assigned duties and avoids excessive and/or unauthorized overtime.

· Communicates the mission, ethics and goals of the hospital as well as the mission and vision of Home Care.

· Supports necessary changes made and is not detrimental to the harmony of the department.

· Uses time care accurately.

· Adheres to dress code and is professional in appearance and conduct.

· Is available when “on-call” according to policy.

Education/Regulatory Requirements & Experience:

· Current LVN/LPN licensure in State of Oklahoma required.

· Minimum of 1-year experience as LPN preferred.

· Previous work history that demonstrates steady attendance and punctuality is required

· Current BLS required.

· Maintains current automobile insurance, current drivers license, and maintains driving record of good standing.

· CPR (BLS) can be obtained during orientation.

Language Skills:

· Ability to read and communicate effectively in English.

· Additional languages preferred.

Skills:

· Basic computer knowledge.
