Job Summary:

A member of patient care team within the Department of Home Health and directly accountable to Patient Clinical Manager. The position requires that the individual assist the professional staff by performing specific nurse aide functions related to patient care to include bathing, shampooing, ambulation, transfers, tidy sick area, skin assessment, and other duties as identified on care plan under the supervision of a registered nurse or therapist. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. The CNA prepares documentation of patient care in the home at the time of visit as applicable and submits documentation in a timely manner according to policy. Notifies RN/Therapists of any changes in patient and assists in meeting needs. CNA participates in in-service and competency programs as applicable. Performs other job duties as assigned.

Performance Expectations:

• Demonstrates ability to bathe patient in shower or tub using appropriate water temperature, preventing falls, and maintain privacy.
• Demonstrates ability to transfers patients safely utilizing safe body mechanics for yourself and patient.
• Demonstrates knowledge of changes in patient that require notification of nurse/therapist.
• Demonstrates ability to document in the EMR according to policies and procedures.

Qualifications:

Education/Skills

High School Diploma/GED required. Completion of an approved Nurse Aide training course required.

Work Experience:

Nurse aide work experience preferred.

Certification/Licensure-DUE UPON HIRE

• Valid Driver's License with proof of Automobile Insurance.

• Certified Nurse Aid Certificate or successful completion of state-approved certification curriculum within 60 days of hire.

Additional Certification/Licensure - obtained based on the required timeframe below
