JOB SUMMARY
The Diabetes Educator assesses patients, develops and implements individualized plans of care, and evaluates patient progress. This role provides education to patients and families to support effective diabetes self-management.
In collaboration with leadership and interdisciplinary teams, the Diabetes Educator coordinates care and education services in alignment with the mission, vision, and values of Campbell County Memorial Hospital (CCH) and Campbell County Clinic. The role follows applicable professional standards and regulatory guidelines for diabetes education and dietetics practice.
ESSENTIAL FUNCTIONS
- Conducts comprehensive patient assessments and accurately documents findings in the medical record.
- Develops individualized, age-appropriate care plans addressing physical, psychosocial, and spiritual needs in collaboration with the interdisciplinary care team.
- Implements and coordinates the plan of care, delegating appropriately and ensuring timely and accurate documentation of interventions.
- Evaluates patient progress toward outcomes, revises care plans as needed, and initiates appropriate interventions.
- Provides diabetes education to patients and families regarding disease process, treatment plans, self-management strategies, and expected outcomes.
- Delivers community education programs, classes, and outreach related to diabetes prevention and management.
- Ensures excellence in customer service for patients, families, and all internal/external stakeholders.
- Maintains clinical competency and adheres to professional standards of practice, continuing education, and regulatory requirements.
- Demonstrates fiscal responsibility and accountability in the delivery of care and use of resources.
- Actively participates in hospital committees, Continuous Quality Improvement (CQI) initiatives, and organizational quality programs.
- Maintains a clean, safe, and therapeutic environment for patients and staff.
- Complies with all aspects of the Corporate Compliance Program, including the Code of Conduct, applicable laws and regulations, and organizational policies and procedures.
- Maintains eligibility to participate in federal and state healthcare programs (i.e., free from exclusion or sanctions).
- Performs other related duties as assigned.
JOB QUALIFICATIONS
- Education
- Graduate of an accredited school of nursing or an accredited Registered Dietitian program.
- Licensure & Certification
- Current Wyoming Registered Nurse (RN) license OR current registration with the Commission on Dietetic Registration and Wyoming Dietetics Licensing Board.
- Current or eligible for certification as a Certified Diabetes Care and Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education.
- Current CPR certification in accordance with organizational policy.
- Experience
- Prior experience in diabetes education, endocrinology, nutrition counseling, chronic disease management, or patient education preferred.
- Clinical experience in acute care, outpatient, or community health settings strongly preferred.
