Job Description

Position Summary

The Licensed Practical Nurse (LPN) provides basic clinical nursing care to patients under the direction and supervision of a Registered Nurse (RN) and/or licensed medical provider. The LPN delivers patient care across the lifespan—infant through geriatric—in a high-volume outpatient setting. This role requires strong clinical skills, effective communication, and the ability to collaborate with a multidisciplinary team to ensure quality, patient-centered care.

Responsibilities and Duties

  • Deliver holistic nursing care to patients of all ages, including medication administration (oral, injectable, and topical), venipuncture, and treatment under provider orders.
  • Conduct patient triage by collecting vital signs (pulse, temperature, blood pressure, height, weight) and recording medical histories.
  • Maintain up-to-date, accurate patient records and documentation in the electronic health record (EHR) system.
  • Communicate effectively with medical staff and patients to ensure continuity of care.
  • Educate patients and their families on diagnoses, treatment plans, medications, and preventive health measures.
  • Uphold clinical standards and follow established protocols, policies, and procedures.
  • Participate in case management, patient follow-up, and coordination of care with internal and external providers.
  • Assist the Director of Nursing with staff orientation, training, and clinical competencies for new and current employees.
  • Participate in Continuous Quality Improvement (CQI) activities and assist in corrective action plans as needed.
  • Monitor and maintain quality assurance processes including medical record audits, standing orders, emergency equipment checks, and clinical supply inventory.
  • Respond to patient complaints and resolve clinical issues in collaboration with the DON and Chief Medical Officer (CMO).
  • Support accreditation readiness and grant compliance by following documentation standards and contributing to reporting requirements.
  • Attend mandatory meetings, participate in committees, and engage in professional development and in-service education.
  • Ensure proper handling, storage, and documentation of vaccines in compliance with VFC, GRITS, and provider agreement requirements.
  • Support organizational goals and strategic planning efforts under the Neighborhood Improvement Project, Inc.
  • Perform other duties as assigned within the scope of licensure.

Qualifications:

Knowledge, Skills, and Abilities:

  • Proficient in clinical procedures, including medication administration and phlebotomy.
  • Ability to apply nursing principles in a culturally competent, patient-centered manner.
  • Strong communication, interpersonal, and team collaboration skills.
  • Knowledge of clinical documentation standards and familiarity with EHR systems.
  • Capacity to multitask and remain calm in high-pressure or emergency situations.
  • Commitment to quality improvement and continuous learning.
  • Knowledge of GRITS, VFC, OSHA, HIPAA, and other regulatory requirements.

Education:

  • Graduate of an accredited Licensed Practical Nurse program.
  • Current and unrestricted LPN license in the State of Georgia or South Carolina (as applicable).
  • CPR certification required.

Experience:

  • Minimum of one (1) year clinical experience in a primary care, urgent care, or outpatient healthcare setting preferred.
  • Experience in administering immunizations and managing vaccines is a plus.

Physical Demands:

  • Full range of motion including handling and lifting patients, manual dexterity, and eye-hand coordination.
  • Requires standing, walking, bending, and lifting items up to 50 pounds.
  • Occasional exposure to infectious diseases, blood and body fluids, and other medical waste.
  • Must be able to work under physically and emotionally stressful conditions in a clinical environment.

Working Conditions:

  • Outpatient clinical environment with frequent interaction with patients, providers, and staff.
  • May require flexible hours, including occasional evening or weekend coverage.
  • Frequent exposure to noise, interruptions, and varied clinical demands.
