Based in Jacksonville, FL

Your Role as a Home Health Aide (HHA/CNA):

  • Provide personal care, bathing, and hands-on care as outlined in the patient’s plan of care.
  • Complete visit records promptly and accurately according to Agency policy.
  • Report any changes in the patient’s condition to the supervising RN.
  • Perform essential household tasks to support health care in the home as assigned.
  • Assist patients with ambulation and exercises as directed by the care plan.

    • Qualifications:

  • Successfully completed a Home Health Aide training program and competency evaluation as required by Medicare Conditions of Participation.
  • Demonstrates a compassionate and caring attitude toward the sick and elderly.
  • Ability to follow directions, read, and write effectively to carry out assigned tasks.
