About This Position

JOB SUMMARY
-
Coordinates the inpatient admission process into Good Shepherd’s facilities. Serves as the primary point of contact for completing admission paperwork with patients and families.

ESSENTIAL FUNCTIONS
-
INITIATES RESPONSE TO NEW REFERRALS
-
By promptly responding to Allscripts referral sources by selecting "Referral Received"
-
By promptly entering demographic information into Cerner pre-admit status according to accepted standards
-
By forwarding referral information for review
-
-
MAINTAINS PRE-ADMISSIONS DATABASE (THE BLUE BLOOK)
-
By entering preliminary patient information
-
-
INITIATES THE ADMISSION PROCESS
-
By scanning admission documents to appropriate unit in a timely manner
-
By greeting and welcoming patients upon arrival
-
By verifying patient identity through accepted standards
-
By assisting admissions department with entering preliminary data information into IRF PAI in UDS for each admitted patient within one business day after admission
-

COMPLETES ADMISSION FOLLOW UPS
-
By following up with patients the next business day and completing admission paperwork with patient and or family via face to face or by phone as appropriate
-
By answering any questions related to admission paperwork and insurance information
-
Prepare and upload admission follow up documents and upload in patients file in cerner
-

COORDINATES DIALYSIS SET UP
-
By setting up dialysis appointments for incoming admissions.
-
Coordinate with dialysis and transportation company to ensure patients appointments run smoothly and timely.

MAINTAINS OFFICE SUPPLIES INVENTORY

By checking stock to determine inventory level
-
By anticipating needed supplies
-
By placing and expediting orders for supplies
-
By verifying receipt of supplies
-
