Nurse Navigator - Clinic in San Angelo, Texas at Shannon Health

Job Summary

The Pain Management Nurse Navigator collaborates with other health care professionals to facilitate information regarding the procedure to ensure the best possible experience for the patient from the clinic to the actual procedure date. Conducts a health history and provides educational instructions to patients prior to the procedure date. Will be the point of contact for the patient throughout the continuum of care. Follow up with patients on a regularly defined intervals after treatment.

Performance: Position Specific Essential Functions

• Utilizes the nursing process of assessment, planning, implementation, and evaluation of the patient according to the provider orders and establishes a plan of care.
• Coordinate and communicate with all members of healthcare team involved in providing patient care.
• Facilitate timely access to quality medical care from diagnosis through procedure and back to clinic (if needed).
• Performs clerical duties to include ensure all pre-authorizations are completed prior to scheduling patients.
• Collaborates with providers and other healthcare disciplines as needed to improve patient care.
• Educate patient and family members on diagnosis, treatment options, and available services. Provide direction and support to the patient/family and serve as patient advocate.
• Maintain accurate and complete nursing documentation.
• Be a point of contact for the patient throughout the continuum of care.
• Participate in continuous quality improvement projects in order to better serve the patient, family, and healthcare system.
• Greets patients and families. Assist by providing facility information, directions, and general information (transportation, hotels, etc.).
• Performs other duties as assigned.

Qualifications

Education

• Required
  • High School Diploma, GED, or equivalent
  • Associate’s degree in nursing

Experience:

• Preferred
  • Three years of Surgical, Clinic, or Department-Specific experience

Certification/Licensure:

• Required
  • Registered Nurse (RN), with authorization to practice in the State of Texas
  • Basic Life Support (BLS) Certification
    • Must obtain within ninety (90) days of start date
