RWJBarnabas Health is seeking a Post-Acute Care Hospitalist Medicine APP in New Jersey!

RWJBarnabas Health is seeking a Subacute Hospitalist to support our Skilled Nursing Facilities in Mercer County, New Jersey.

The Subacute Hospitalist APP will manage post-acute and chronic diseases, diagnose and monitor medical conditions and illnesses, and implement patient care plans. They will also collaborate and coordinate care with other advanced practice providers, physicians, facility leaders, ancillary services, and support staff.

Ideal candidates will have a strong work ethic and a desire to participate in a team-oriented busy practice. They will also engage with sub-specialists, case managers, social workers, and all others involved in the patients' care, discharge planning, and patient/family communications.

Candidate Requirements:

• • Physician Assistant Certification
  • Registration & Licensure by the New Jersey State Board of Medical Examiners/ Physician Assistant Advisory required

OR

• • Master’s degree in Nursing
  • NP Certification
  • Registered Professional Nurse with current NJ state license; CDS and DEA.
• Prior experience as an APP working as a Hospitalist or Extensivist in SNF preferred
• Palliative / Hospice experience preferred
• Basic Life Support (BLS) and Advanced Life Support (ACLS) Certifications, PALS (if required)
• Active membership in a professional organization preferred
• Ability and willingness accommodate scheduling needs including working extended hours, weekdays, overnights, weekends, and holidays
• No procedures required
